Służyć temu będzie wzmocnienie koordynacji opieki nad pacjentem, pacjenci będą otrzymywali zwiększony dostęp do pomocy w utworzonym dziennym oddziale psychogeriatrycznym, ale też projekt zakłada rozwój środowiskowych form opieki i utworzenie zespołu środowiskowego.
