Projekt zaplanowano od 01.06.2019 r.do 31.05.2021r. w Częstochowie i powiecie: częst.,myszk.,kłob (sub. pół. woj. śl.). Grupę docelową stanowi 35 osób dorosłych (18K) z niepełnosprawnością w stopniu umiarkowanym, znacznym lub sprzężoną oraz 12 osób z otoczenia ON (10K). Celem projektu jest wzrost zdolności do podjęcia zatrudnienia wskazanych powyżej osób poprzez realizację usług aktywnej integracji. W projekcie zaplanowano opracowanie indywidualnej ścieżki reintegracji, a następnie realizację wsparcia, w ramach innowacyjnego Studium społ.-zawod. (seminaria, warsztaty),utworzenie mieszkania wspomagano-treningowego, wsparcie społ.-zawod.dla otoczenia ON. Wskaźniki: - l.os. zagr. ubóst. lub wykl. społ. objętych wsparciem -47(28K), - l.os. z niep. objętych wsparciem -35(18K), - l.os. zagrożonych ubóstwem lub wykluczeniem społecznym, które: uzyskały kwalifikacje po programie -21(11K),poszukują pracy po opuszczeniu programu -12(6K),pracują po opuszczeniu programu -10(5K).
