The project was developed in partnership with the Association “Promotion of Entrepreneurship” (SPP) and Strzelce Op. (Social Assistance Centre). The project covered the following districts: Kędzierzyńsko-kozielski, shooting and krapkowicki. The need to implement the project results from the demographic situation in the Opole Voivodship, i.e. the increase in the number of older people, including disability, at risk of social exclusion, i.e. people in the area. 60 years of age (according to the statistics in the above poviats in 2014, the majority were women – on average 58 %), which are often handled by actual carers and those who do not require the care of an actual carer, who suffer from cardiovascular diseases, apnea, pain syndromes, and do not have access to high-quality health, rehabilitation and care services, improving their health status, functionality and limiting premature mortality. The tasks planned in the project aim to bridge the above barriers. The project envisages tasks: the separation of 500 elderly people (participants of the project), including 60 % at risk of social exclusion, providing medical care in place for actual carers in order to temporarily relieve them from their daily responsibilities (18 people), training for 18 actual carers, including psychological support, in order to improve qualifications for care, medical tele-care for 500 participants of the project, rehabilitation services for 300 participants and provision of transport for rehabilitation for 128 participants. It is also planned to provide specialised rehabilitation beds for 3 patients. The project will contribute to achieving the main objective of enabling access to high-quality health services for elderly people at risk of social exclusion, thereby giving them the chance to extend and improve their quality of life, to live without pain with greater physical fitness.     Projekt został opracowany w partnerstwie ze Stowarzyszeniem „Promocja przedsiębiorczości” (SPP) oraz Gminą Strzelce Op. (Ośrodkiem Pomocy Społecznej). Projektem objęto powiaty: kędzierzyńsko-kozielski, strzelecki oraz krapkowicki. Potrzeba realizacji projektu wynika z sytuacji demograficznej w woj. opolskim, tj. wzrostem liczby osób starszych, w tym z niepełnosprawnościami, zagrożonych wykluczeniem społecznym, tj. osób pow. 60 roku życia (wg danych stat. w ww powiatach w roku 2014 w większości były to kobiety – średnio 58%), którymi niejednokrotnie zajmują się opiekunowie faktyczni oraz tych, którzy nie wymagają opieki opiekuna faktycznego, a u których występują choroby układu krążenia, bezdech, zespoły bólowe, a nie mają oni dostępu do wysokiej jakości usług zdrowotnych, rehabilitacyjnych i pielęgnacyjnych, poprawiających ich stan zdrowia, funkcjonalność oraz ograniczających przedwczesną umieralność. Zadania zaplanowane w projekcie mają na celu zniwelowanie powyższych barier. W projekcie przewidziano zadania: wyodrębnienie 500 os. grupy docelowej osób starszych (uczestników projektu), w tym 60% zagrożonych wykluczeniem społecznym, zapewnienie opieki medycznej w zastępstwie za opiekunów faktycznych w celu czasowego ich odciążenia od codziennych obowiązków (18 osób), szkolenia dla 18 opiekunów faktycznych, w tym wsparcie psychologiczne, w celu podniesienia kwalifikacji do opieki, teleopieka medyczna dla 500 uczestników proj., usługi rehabilitacyjne dla 300 uczestników proj. oraz zapewnienie transportu na rehabilitację dla 128 uczestników proj. oraz transport ambulatoryjny (10 osób). Przewidziano także udostępnienie dla 3 pacjentów specjalistycznych łóżek rehabilitacyjnych. Projekt przyczyni się do osiągnięcia celu głównego, jakim jest umożliwienie dostępu do wysokiej jakości usług zdrowotnych dla osób starszych, zagrożonych wykluczeniem społecznym, a tym samym danie im szansy na wydłużenie i poprawę jakości ich życia, życia bez bólu z większą sprawnością fizyczną.
