W ramach projektu na terenie Gminy Sokółka 90 os (60K i 30M) w wieku 60 + kobiety oraz 65+ mężczyźni zostanie objętych kompleksowym wsparciem w postaci usług opiekuńczych oraz otrzyma dostęp do centrum teleopieki. Celem będzie zorganizowanie kompleksowego systemu opieki, umożliwiającego osobom niesamodzielnym jak najdłuższe bezpieczne pozostanie w ich środowisku oraz miejscu zamieszkania. Formy wsparcia przewidziane w projekcie:Usługi opiekuńcze: zatrudnienie 5 opiekunek (umowa o pracę pełen etat). Uczestnicy projektu, według potrzeb otrzymają wsparcie w formie usług opiekuńczych w wymiarze160 godz. opieki na opiekunkę w miesiącu przy czym liczba godzin w poszczególnych dniach, jak również w przypadku poszczególnych osób, może być różna i będzie wynikać z decyzji adm ośrodka pomocy społecznej. W ramach świadczonych usług opiekuńczych Uczestnicy projektu będą mogli skorzystać z pomocy w zachowaniu higeny osobistej, pomocy w wykonywaniu czynności życia codziennego, wsparcia emocjonalnego. Opiekunki będą świadczyć usługi opiekuńcze równolegle u tych samych uczestników u których zainstalowany zostanie system TELEOPIEKI. Usługi opiekuńcze świadczone w ramach projektu dotyczyć będą osób nowych. Teleopieka jest to usługa wykorzystująca nowoczesne technologie informacyjno-komunikacyjnych oraz nowe modele organizacji udzielania pomocy. Umożliwia wezwania pomocy w przypadku nagłych zdarzeń wymagających interwencji opiekuna, zagrożenia bezpieczeństwa lub zdarzeń medycznych, które są problematyczne lub niebezpieczne. Pomoc wzywana jest przy użyciu urządzania alarmowego i komunikacyjnego posiadającego przycisk SOS. Całodobowe Centrum Telemedyczne organizuje i zarządza procesem udzielania pomocy aktywując odpowiednie służby na terenie powiatu, lub przekazując opiekunkom odpowiedzialnym za danego podopiecznego informacje o konieczności interwencji. Usługi opiekuńcze wraz ze wsparciem Teleopieki pozwalają realizować założenia projektowe w sposób indywidualny i kompleksowy. (Polish)

As part of the project in the municipality of Sokółka 90 people (60K and 30M) aged 60 + women and 65+ men will receive comprehensive support in the form of care services and will receive access to the telecare centre. The aim will be to organise a comprehensive care system that will allow dependants to remain as safe as possible in their environment and in their place of residence. Forms of support provided for in the project: Care services: employment of 5 carers (full-time employment contract). The project participants will receive support in the form of care services of 160 hours per month as required, with the number of hours per day, as well as for individuals, varying and will result from the decision of the social assistance centre adm. As part of the care services provided, participants of the project will be able to benefit from assistance in maintaining personal higene, assistance in performing everyday activities, emotional support. Caregivers will provide care services in parallel with the same participants in which the tele-care system will be installed. The care services provided under the project will apply to new people. Telecare is a service utilising modern information and communication technologies and new models of organisation of assistance. It enables emergency calls for emergency situations requiring caregiver intervention, safety hazards or medical events that are problematic or dangerous. Help is requested using an alarm and communication device with an SOS button. A -hour telemedicine centre organises and manages the assistance process by activating the relevant services in the county, or providing the caregivers responsible for the caregivers with information on the need for intervention. Care services together with telecare support allow to implement design assumptions in an individual and comprehensive manner. (English)
