Holy Trinity in Gorzów Śl. The scope of construction works includes the reconstruction, expansion and superstructure of the former monastery of the Elżbietanek Sisters in Gorzów Śląski, ul. Kluczborska 15 with the creation of a communication system. After completion of the construction works, the facility will be provided with equipment enabling the establishment of the Caritas Care Station and a team of rehabilitation offices (in the ground floor) and the Daily Senior House providing services to the elderly (20 participants), especially those most at risk of social exclusion due to their property status and family status. The scope of multifunctional activities of the centre will include, among other things, the provision of care services directly at the place of residence of the needy, activation activities, rehabilitation and other assistance activities of the nature of social services implemented in the local environment for those at risk of exclusion and the poor. Przedmiotem projektu jest kompleksowe przedsięwzięcie inwestycyjne (projekt rewitalizacyjny) wynikający z Lokalnego-Programu-Rewitalizacji-Gminy-Gorzów-Sląski-na-lata-2016-2020, które będzie się przyczyniać do aktywizacji środowisk ubogich i zagrożonych wykluczeniem społecznym. Projekt będzie realizowany w partnerstwie na podstawie zawartej Umowy o partnerstwie na rzecz realizacji Projektu; jego Liderem jest Caritas Diecezji Opolskiej a Partnerem Parafia Rzymsko-Katolicka pw. Trójcy Świętej w Gorzowie Śl. Zakres robót budowlanych obejmuje przebudowę, rozbudowę i nadbudowę budynku dawnego klasztoru sióstr Elżbietanek w Gorzowie Śląskim, ul. Kluczborska 15 wraz ze stworzeniem układu komunikacyjnego . Po zakończeniu prac budowlanych obiekt zostanie zaopatrzony w wyposażenie pozwalające na podjęcie działalności Stacji Opieki Caritas i zespołu gabinetów rehabilitacyjnych (w kondygnacji parteru) oraz Dziennego Domu Seniora świadczącego usługi na rzecz osób w podeszłym wieku (20 uczestników), szczególnie tych najbardziej zagrożonych wykluczeniem społecznym ze względu na status majątkowy i stan rodzinny. Zakres działań wielofunkcyjnego ośrodka obejmować będzie m. in. świadczenie usług opiekuńczych bezpośrednio w miejscu zamieszkania potrzebujących, zajęcia aktywizacyjne, świadczenia rehabilitacyjne i inne działania pomocowe o charakterze realizowanych w środowisku lokalnym usług społecznych kierowanych do osób zagrożonych wykluczeniem i ubogich. Dzięki realizacji projektu rewitalizacją objęty zostanie obszar 0,11 ha (1 136 m2, 0,08% udziału w odniesieniu do obszaru objętego programem rewitalizacji), wsparty zostanie 1 obiekt infrastruktury zlokalizowany na rewitalizowanych obszarach (ramy wykonania), 1 obiekt zostanie dostosowany do potrzeb osób z niepełnosprawnościami, 1 do świadczenia usług w zakresie opieki nad osobami zależnymi, w 1 projekcie sfinansowano koszty racjonalnych usprawnień dla osób z niepełnosprawnościami.
