Construction of the Senior House in Pawłowice will be carried out in the Northern Subregion, in the commune of Gorzów Śląski, Pawłowice on plots no 416/1, 416/2, 417/3. The investment will include: — the construction of a two-storey complex with no architectural barriers, in which two independent establishments will be created, i.e. a -hour residence for a maximum of 30 inhabitants, including 7 people in heavy condition and apartments supported for up to 25 people, – reconstruction of the farm building into an ecumenical chapel and its equipment, – the installation of the area i.e. a garden for recreation and recreational activities together with a mini zoo, walking alleys, outdoor parking spaces, games, etc. The applicant’s intention is to create an exceptional and distinctive social services facility in Opole voivodship, intended for the -hour stay of elderly, chronically ill and disabled people. Budowa Domu Seniora w Pawłowicach realizowana będzie na obszarze Subregionu Północnego, w gminie Gorzów Śląski, miejscowości Pawłowice na działkach nr 416/1, 416/2, 417/3. Inwestycja obejmować będzie: - wzniesienie dwukondygnacyjnego kompleksowo wyposażonego obiektu pozbawionego barier architektonicznych, w którym zostaną utworzone dwie niezależne placówki tj. dom całodobowego pobytu dla maksymalnej liczby 30 mieszkańców, w tym 7 osób w stanie ciężkim oraz mieszkania wspierane dla maksymalnie 25 osób, - przebudowę budynku gospodarczego na kaplicę ekumeniczną oraz jej wyposażenie, - zagospodarowanie terenu tj. urządzenie ogrodu służącego do wypoczynku i zajęć rekreacyjnych wraz z mini zoo, alejami spacerowymi , miejscem do grillowania, miejscami wyposażonymi w stoły do gry w szachy, tężnią solną, miejsca parkingowe, instalacja oświetlenia zewnętrznego, ogrodzenie terenu. Zamiarem Wnioskodawcy jest stworzenie wyjątkowego oraz wyróżniającego się w skali województwa opolskiego obiektu usług społecznych, z przeznaczeniem na całodobowy pobyt osób starszych, przewlekle chorych i niepełnosprawnych. W ramach realizowanego projektu osiągnięte zostaną następujące wskaźniki produktu: - Liczba obiektów dostosowanych do potrzeb osób z niepełnosprawnościami-1szt. - Liczba projektów, w których sfinansowano koszty racjonalnych usprawnień dla osób z niepełnosprawnościami - 1szt. - Liczba wspartych ośrodków opieki nad osobami zależnymi - 2szt. - Liczba wspartych placówek zapewniających całodobową opiekę - 1szt. - Liczba wspartych mieszkań wspieranych/treningowych - 1szt. -Liczba miejsc utworzonych w placówkach zapewniających całodobową opiekę - 30szt. - Liczba miejsc utworzonych w mieszkaniach wspieranych/treningowych - 25szt.
