The project will be implemented in the period from 01.10.2017-30.09.2019 for the period of 24 months. The aim of the project is to: Return to active function. social and incl. 80 people at risk of poverty or social exclusion, residents in the Municipality of M.Toruń based on the use of PAL activities and increasing awareness of the available forms of assistance in 40 members of these people’s environment by 30.09.2019 The project is aimed at 120 people, including 80 people at risk of poverty or exclusion. (unemployed persons qualified for third profile (32 persons), unemployed persons qualified for I and II profiles, disabled persons, inactive persons and others) and 40 people from the environment of people at risk. The project prefers to include people from degraded areas, i.e. Podgórze, Bydgoski Przedmieście and Old Town. The project will carry out individualisation and comprehensive activities enabling active social inclusion, as well as a return to the labour market through a social contract and the Local Activity Programme. Within the project, actions will be undertaken with the participation of professional, educational, social and health activation instruments. The process of supporting people, families and environments at risk of poverty or social exclusion will be based on a path of reintegration, created individually for each person, family, environment at risk of poverty or social exclusion, taking into account the diagnosis of problem situations, resources, potential, predisposition and needs. Support within the project is focused on the person and their needs, not on the development of services. The project takes into account the situation on the local labour market. MOPR, who is the project developer, works closely with PUP for MT. The project assumes a social efficiency of 34 % and employment efficiency of 22 %. Projekt będzie realizowany w okresie od 01.10.2017-30.09.2019 r. przez okres 24 m-cy. Celem projektu jest: Powrót do aktywnego funkcj. społ. i zaw. 80 osób zagroż. ubóstwem lub wykluczeniem społ., zamieszk. na terenie Gminy M.Toruń w oparciu o zastosowanie działań PAL oraz zwiększenie umiej. opiekuńczych i świadomości nt. dostępnych form pomocy u 40 członków otoczenia tych osób do 30.09.2019 r. Proj. skierowany jest do 120 osób, w tym 80 osób zagrożonych ubóstwem lub wyklucz. społ. (osoby bezrobotne zakwalifikowane do III profilu (32 os.), osoby bezrobotne zakwalifikowane do I i II profilu, osoby niepełnosprawne, osoby bierne zawodowo i in.) oraz 40 osób z otoczenia osób zagroż.wykluczeniem społ. Projekt prefer. do objęcia wspar. osoby pochodz. z obszarów zdegradowanych, objętych rewitalizacją tj. z Podgórza, Bydgoskiego Przedmieścia oraz Starego Miasta. W proj. prowadzone będą zindywidualizowanie i kompleksowe działania umożliwiające aktywne włączenie społeczne, a także powrót na rynek pracy realizowane poprzez kontrakt socjalny oraz Program Aktywności Lokalnej. W ramach proj. będą podejmowane działania z udziałem instrumentów aktywizacji zawodowej, edukacyjnej, społecznej oraz o charakterze zdrowotnym. Proces wsparcia osób, rodzin i środowisk zagrożonych ubóstwem lub wykluczeniem społecznym odbywać się będzie w oparciu o ścieżkę reintegracji, stworzoną indywidualnie dla każdej osoby, rodziny, środowiska zagrożonych ubóstwem lub wykluczeniem społecznym, z uwzględnieniem diagnozy sytuacji problemowej, zasobów, potencjału, predyspozycji i potrzeb. Wsparcie w ramach proj. koncentrowane jest na osobie i jej potrzebach, nie zaś na rozwijaniu usług. Proj. uwzględnia sytuację na lokalnym rynku pracy. MOPR będący realizatorem projektu ściśle współpracuje z PUP dla MT. Proj. zakłada efektywność społ. na poziomie 34% oraz efektywność zatrudnieniową na poziomie 22%. Realizowany proj. jest zgodny z Krajowym Programem Przeciwdziałania Ubóstwu i Wykluczeniu Społecznemu 2020. (Polish)
