The subject of this project will be based primarily on investment tasks related to carrying out construction works consisting of the reconstruction (including overbuilding and adaptation of the premises) of the Main Building in the SPZOZ in Strzelno in order to adapt it to the needs of the Care and Medical Department. This is necessary from the point of view of achieving the main objective of the project, which is to improve the quality and availability of long-term care in the Kujawsko-Pomorskie Voivodeship (including in particular the Pomeranian district) by creating new places of care in the applicant’s structures. The material scope of the project to be implemented includes the following activities:1. Preparation of the project (preparation costs: Feasibility Study and Functional and Useful Programme and Construction and Contractual Project for the planned construction investment).2. Construction works (costs associated with carrying out construction works consisting of rebuilding the Main Hospital Philistine Building in Strzelno under the needs of an enlarged ZOL).3. Purchase of medical equipment (costs related to the purchase of 166 pieces of medical equipment, which will be upgraded to ZOL).4. Project management (costs related to carrying out investor supervision of construction works and project coordination by the Project Team composed of staff seconded to work on the project staff of the Hospital – remuneration of project coordinator, project accounting officer, employee for tenders, promotion and monitoring of the project).5. Project promotion (costs related to information and promotion activities planned for the benefit of this project). 18.09.2017 (then the first contract related to the proposed project was signed, which concerned the preparation of a feasibility study of the investment). Przedmiot niniejszego projektu opierać się będzie przede wszystkim na zadaniach inwestycyjnych związanych z przeprowadzeniem prac budowlanych polegających na przebudowie (w tym także nadbudowie i adaptacji pomieszczeń) Budynku Głównego w Filii SPZOZ w Strzelnie w celu dostosowania go pod potrzeby Zakładu Opiekuńczo-Leczniczego. Jest to niezbędne z punktu widzenia realizacji celu głównego projektu, którym jest podniesienie jakości i dostępności do opieki długoterminowej na terenie woj. kujawsko-pomorskiego (w tym przede wszystkim powiatu mogileńskiego) poprzez utworzenie nowych miejsc tej opieki w strukturach Wnioskodawcy. Zakres rzeczowy planowanego do realizacji przedsięwzięcia obejmuje następujące działania:1. Przygotowanie projektu (koszty związane z przygotowaniem: Studium Wykonalności oraz Programu Funkcjonalno-Użytkowego i Projektu budowlano-wykonawczego pod planowaną inwestycję budowlaną).2. Roboty budowlane (koszty związane z przeprowadzeniem prac budowlanych polegających na przebudowie Budynku Głównego Filii Szpitala w Strzelnie pod potrzeby powiększonego ZOL-u).3. Zakup aparatury medycznej (koszty związane z zakupem 166 szt. sprzętu medycznego, którymi doposażony zostanie rozbudowany ZOL).4. Zarządzanie projektem (koszty związane z prowadzeniem nadzoru inwestorskiego nad robotami budowlanymi oraz z koordynacją projektu przez Zespół Projektowy złożony z oddelegowanych do pracy przy projekcie pracowników Szpitala - wynagrodzenia koordynatora projektu, pracownika ds. obsługi księgowej projektu, pracownika ds. przetargów, promocji i monitoringu projektu).5. Promocja projektu (koszty związane z planowanymi do przeprowadzenia na rzecz niniejszego projektu działaniami informacyjno-promocyjnymi).Za datę rozpoczęcia projektu przyjęto 18.09.2017 r. (wówczas nastąpiło podpisanie pierwszej umowy związanej z wnioskowanym projektem, która dotyczyła przygotowania studium wykonalności inwestycji). Zakończenie realizacji inwestycji zaplanowano z kolei na 31.08.2020 roku. (Polish)
