The project aims to improve access to care services for dependants living in the Jedlina Zdrój commune during the period of 24 m by: professional activation of the 8 most distant people from the roar and preparing them to work as a caregiver for dependants, the mobilisation of 6 places to provide care services (6 carers) for 14 new groups/independent persons, at risk of social exclusion and poverty, including social activation of carers of genuine dependants – 20 by providing psychological and animation support. The project aims to achieve a social-employment efficiency ratio of at least 56 % – 16 people, and at least 22 % – 2 person in employment. 8 professional activation, 14 care services and 20 social activation – 35 K and 7 M. The project will be implemented in partnership with a specialised entity with the status of training institution and employment agency, with 12 years of experience in working with people at risk of social exclusion and poverty. Support for dependants will be provided in accordance with "the general European guidelines for the transition from institutional care to community-based care and the document "Using European Union funds to move from institutional care to community-based care – a toolbox”. Projekt zakłada poprawę dostępu do usług opiekuńczych dla osób niesamodzielnych zamieszkałych na terenie gminy Jedlina Zdrój w okresie 24 m-cy poprzez: aktywizację zawodową 8 osób najbardziej oddalonych od ryku i przygotowanie ich do pracy w zawodzie opiekuna osób niesamodzielnych, uruchomienie 6 miejsc świadczenia usług opiekuńczych (6 opiekunów) dla 14 nowych środowisk/ osób niesamodzielnych, zagrożonych wykluczeniem społecznym i ubóstwem, objęcie działaniami aktywizacji społecznej opiekunów faktycznych osób niesamodzielnych - 20 osób poprzez objęcie ich wsparciem psychologicznym i animacyjnym. Projekt zakłada osiągnięcie wskaźnika efektywności społeczno – zatrudnieniowej w wymiarze społecznym co najmniej 56% - 16 osoby, oraz w wymiarze zatrudnieniowym co najmniej 22% - 2 osoba.Łącznie wsparciem w ramach projektu objętych zostanie42 osób przy czym: 8aktywizacją zawodową, 14 usługami opiekuńczymi i 20 aktywizacją społeczną – 35 K i 7 M. Projekt realizowany będzie w partnerstwie z wyspecjalizowanym podmiotem posiadającym status instytucji szkoleniowej i agencji zatrudnienia, posiadającej 12 letnie doświadczenie w pracy z osobami zagrożonymi wykluczeniem społecznym i ubóstwem. Wsparcie osobom niesamodzielnym będzie udzielane zgodnie z „Ogólnoeuropejskimi wytycznymi dotyczącymi przejścia od opieki instytucjonalnej do opieki świadczonej na poziomie lokalnych społeczności i dokumentem „Wykorzystanie funduszy Unii Europejskiej w celu przejścia od opieki instytucjonalnej do opieki świadczonej na poziomie lokalnych społeczności – zestaw narzędzi”.
