The project contributes to the specific objective of ROP 7.6-2022 will increase the availability of social services, in particular community-based services, care services and family support services for 76 inhabitants of the police district at risk of poverty and/or social exclusion. The project will be carried out in the area of Police, which is included in the list of programmes for the regeneration of the municipalities of the West Pomeranian Region, including the area to be regenerated, which is the day of the publication of the application for funding in the LSI, which rewards 1. As a result, 14 social service sites supported in the programme will be in existence at the end of the project, 14 service places in the sheltered housing set up in the programme will be in existence at the end of the project, 76 people at risk of poverty or social exclusion will leave institutional care for the social services of the local community in the programme. The project is aimed at 60 homeless K/M from Police and 6 mothers with 10 young children. The main tasks of the project are the social reintegration of homeless people, mothers with minor children, the renovation and equipping of sheltered housing, social work. The services provided in the accommodation of the temporary accommodation do not have the character of a rehabilitation tournament. It is a peer-to-peer project to be concluded before the Funding Decision. (English)

    Projekt przewiduje tworzenie 14 nowych miejsc w 5 nowych mieszkaniach chronionych. Mieszkania nie będą zlokalizowane na terenie placówki opieki instytucjonalnej. Tworzone miejsca opieki świadczone będą w społeczności lokalnej. Projekt przewiduje zwiększenie liczby miejsc w mieszkaniach chronionych o 14 w stosunku do stanu na dzień 31 grudnia 2018 r. bez pogorszenia jakości świadczonych usług. Projekt przyczynia się do realizacji celu szczegółowego Działania 7.6 RPO WZ - do 2022 r. zwiększy się dostępność usług społecznych w szczególności usług środowiskowych, opiekuńczych oraz usług wsparcia rodziny dla 76 mieszkańców Powiatu Polickiego zagrożonych ubóstwem i/lub wykluczeniem społecznym. Projekt realizowany będzie na obszarze Gminy Police, która znajduje się na Wykazie programów rewitalizacji gmin województwa zachodniopomorskiego, w tym na obszarze objętym rewitalizacją, aktualnym na dzień opublikowania wniosku o dofinansowanie w LSI - kryt. premiujące 1. W rezultacie 14 miejsc świadczenia usług społecznych wspartych w programie będzie istniało po zakończeniu projektu, 14 miejsc świadczenia usług w mieszkaniach chronionych utworzonych w programie będzie istniało po zakończeniu projektu, 76 osób zagrożonych ubóstwem lub wykluczeniem społecznym opuści opiekę instytucjonalną na rzecz usług społecznych świadczonych w społeczności lokalnej w programie. Projekt jest skierowany do 60 bezdomnych K/M z Gminy Police i 6 matek z 10 małoletnimi dziećmi. Główne zadania projektu to reintegracja społeczna osób bezdomnych, matek z małoletnimi dziećmi, remont i wyposażenie mieszkań chronionych, praca socjalna. Usługi świadczone w mieszkaniach wspieranych pobytu okresowego nie mają charakteru turnusów rehabilitacyjnych. Jest to partnerski projekt dwóch jst-umowa zostanie zawarta przed decyzją o dofinansowaniu. (Polish)
