Projekt „Aktywnie do sukcesu” skierowany jest do 100 osób (K-64, M-36), korzystających z pomocy społecznej zagrożonych wykluczeniem społecznym, ubóstwem, korzystających z POPŻ, bezrobotnych z określonym III profilem, nieaktywnych zawodowo i niepełnosprawnych. Głównym celem projektu jest zaktywizowanie osób zamieszkałych na terenie miasta Łomża zagrożonych ubóstwem i zjawiskiem wykluczenia społecznego poprzez umożliwienie tym osobom dostępu do kompleksowej i indywidualnej oferty mającej na celu ich aktywizację społeczną, zdrowotną, edukacyjną i zawodową. Projekt realizowany będzie w okresie od września 2016 do sierpnia 2018r. Podzielony będzie na 2 Moduły po 50 uczestników. Osoby bezrobotne z określonym III profilem 20 osób będą objęci Programem Aktywizacja Integracja. Program PAI będzie realizowany na podstawie porozumienia z Powiatowym Urzędem Pracy w Łomży który będzie finansował 60% kosztów prac społecznie użytecznych. Dla 28 osób zaplanowano staże. Moduł I będzie realizowany od września 2016 do sierpnia 2017 r. Moduł II od września 2017 do sierpnia 2018r. Uczestnicy projektu będą mogli skorzystać z instrumentów integracji o charakterze społecznym, zawodowym i zdrowotnym, ze wsparcia mediatora, prawnika, psychologa, doradcy zawodowego, terapeuty uzależnień, uczestniczyć w grupie wsparcia. Ponadto nabędą umiejętności w zakresie autoprezentacji, kompetencji pedagogiczno-wychowawczych. W projekcie zaplanowano również warsztaty zarządzania budżetem domowym oraz warsztaty dla uczestników i osób z otoczenia w ramach działania środowiskowego. Dzieci w wieku przedszkolnym będą miały zapewnioną opiekę w przedszkolu i żłobku. Podczas projektu uczestnicy otrzymają catering. W ramach pracy socjalnej będą realizowane kontrakty socjalne. Dla każdego uczestnika projektu zostanie ustalona indywidualna ścieżka reintegracji, uwzględniona zostanie diagnoza sytuacji problemowej, zasoby, potencjał, predyspozycje i potrzeby. (Polish)

	The “Active to Success” project is targeted at 100 persons (K-64, M-36) who are socially vulnerable, are at risk of poverty, who are beneficiaries of PONS, unemployed with a specific post, who are economically inactive and disabled. The main objective of the project is to activate people living in the city of Łomża at risk of poverty and social exclusion by giving them access to a comprehensive and individual offer aimed at their social, health, educational and professional activation. The project will run from September 2016 to August 2018. It will be divided into 2 Modules of 50 participants. The unemployed with a specific III profile of 20 will be included in the Activation Inclusion Programme. The PAI will be implemented on the basis of an agreement with the District Labour Office in Łomża, which will finance 60 % of the costs of socially useful work. For 28 people, traineeships are planned. Module I will run from September 2016 to August 2017 by Module II from September 2017 to August 2018. Participants in the project will be able to benefit from social, professional and health integration instruments, assisted by a mediator, a lawyer, a psychologist, a professional adviser, addiction therapist, to participate in the support group. In addition, they will also acquire the skills of self-presentation, pedagogical and educational competences. The project will also foresee a household budget management workshop and a workshop for participants and for the environment in the framework of the environmental action. Pre-school and crèche facilities will be provided for pre-school children. Participants will receive catering during the project. In the framework of social work, social contracts will be implemented. For each participant in the project, an individual reintegration pathway will be established, a diagnosis of the problem situation, resources, potential, suitability and needs will be taken into account.
