Celem projektu jest zwiększenie dostępności usług społecznych dla 65 osób (35K, 30M) z 15 rodzin zamieszkałych na terenie gminy Rozogi, zagrożonych ryzykiem ubóstwa i/lub wykluczenia społecznego poprzez objęcie ich w okresie 01.10.2017 - 30.09.2018 usługami społecznymi w postaci zadań: indywidualnej diagnozy rodziny, poradnictwa indywidualnego, zajęć grupowych, zajęć wyjazdowych, pracy socjalnej, prowadzącymi do aktywnego udziału w życiu społecznym oraz powrocie na rynek pracy. W ramach projektu realizowane będą następujące zadania: indywidualna diagnoza rodziny, poradnictwo indywidualne, zajęcia grupowe, zajęcia wyjazdowe, praca socjalna. Uczestnikami projektu będzie 65 (35K, 30M) osób z 15 rodzin zamieszkałych na terenie gminy Rozogi w rozumieniu Kodeksu Cywilnego. Uczestnikami projektu, są osoby zagrożone ryzykiem ubóstwa i/lub wykluczenia społecznego (zgodnie z definicją określoną w horyzontalnych, krajowych wytycznych ministra właściwego ds. rozwoju regionalnego dotyczących realizacji CT 9 stworzoną na podstawie Programu Przeciwdziałania Ubóstwu i Wykluczeniu Społecznemu), w tym osoby bezrobotne, które zgodnie z ustawą o promocji zatrudnienia i instytucjach rynku pracy znajdą się w trzeciej grupie osób (tzw. oddalonych od rynku pracy) mieszkające poza Miejskim Obszarem Funkcjonalnym Olsztyna. Wsparciem obejmowane będą rodziny wieloproblemowe. Dzięki realizacji projektu zwiększy się dostępność usług społecznych na terenie gminy Rozogi. (Polish)

The aim of the project is to increase the availability of social services for 65 persons (35K, 30M) with 15 families resident in the municipality of Rozogi, at risk of poverty and/or social exclusion by covering social services in the form of tasks for the period 01.10.2017-30.09.2018: individual family diagnosis, individual counselling, group activities, external activities, social work leading to active participation in society and reintegration into the labour market. The project will carry out the following tasks: family diagnosis, individual counselling, group activities, external activities, social work. The project participants will be 65 (35K, 30M) of persons with 15 families living in a municipality of Rozogi within the meaning of the Civil Code. The project participants are people at risk of poverty and/or social exclusion (as defined in the horizontal national guidelines of the Minister for Regional Development concerning the implementation of CT 9 set up under the Anti-Poverty and Social Exclusion Programme), including unemployed persons who, in accordance with the Act on the promotion of employment and labour market institutions, are located in a third group of people (known as the “labour market”) living outside the City Functional Area of Olsztyn. Support will be provided for multi-problem families. The project will increase the availability of social services in the municipality of Rozogi. (English)
