Projekt zakłada w okresie od 01.01.2020 – do 31.12.2021 zapewnienie 269 osobom ( w tym 173K i 96 M) z zaburzeniami psychicznymi kompleksowej opieki zdrowotnej w najbliższym środowisku pacjenta. Służyć temu będzie wzmocnienie koordynacji opieki nad pacjentem, pacjenci będą otrzymywali zwiększony dostęp do pomocy w utworzonym dziennym oddziale psychogeriatrycznym, ale też projekt zakłada rozwój środowiskowych form opieki i utworzenie zespołu środowiskowego. The project assumes from 01.01.2020 to 31.12.2021 the provision of 269 persons (including 173K and 96M) with mental disorders with mental disorders in the immediate environment of the patient. This will help to strengthen the coordination of patient care, patients will be given increased access to help in the formation of a psychogeriatric branch, but also the project involves the development of community-based care and the creation of an environmental team.
