Cel gł.: Podniesiony poziom aktywności społeczno-zawodowej wśród min 56% os.z GD oraz zwiększona zdolność do podjęcia zatrudnienia wśród 70 os.zagrożonych ubóstwem lub wykluczeniem społ.,zamieszkujących na obsz.woj. mazowieckiego,dzięki kompleksow. wsparciu w postaci usług aktywnej integracji w proj.w okr.01.VI.2017-31.V.2018. Grupę docelową proj.stanowi 70 os.(42 K i 28 M)zagrożonych ubóstwem lub wyklucz.społecz. tj.os.korzystajace ze świadczeń pom.społ zg.z ust.z dn.12.III.2004 o pom.społ.lub kwalifik.się do objęcia wsparciem pom.społ.(tj.spełniające co najmn.jeden z warunków określ.w art. 7 ust.o pom.społ.),os.z niepełnosprawnością(min.50%GD), os.zakwalif.do III profilu pomocy, zg.z ust.z dn.20.IV.2004 o promocji zatrudn.i inst.r.pr.(min.min.20%GD),os.korzystające z PO PŻ (min. 30% GD). Ponadto: -min.90%GD to os.doświadcz.wielokrotn.wyklucz.,zaś min.10%GD to os.o znacznym lub umiark.stopniu niepełnospr. -min.60% GD będą stanowić K. Proj.skierow.jest do GD z terenu woj.mazow. W ramach proj.zostaną podj.zad.: 1.identyfikacja indywidual.potrzeb oraz potencjałów UP; 2.poradnictwo psycholog. i psychospoł., 3.pośrednictwo pracy i poradnictwo zawod.; 4.Trening kompetencji i umiejętności społecz.dla niepełnospr.UP 5.Szkolenia,6.Staże zawodowe. Rezultaty proj.to m.in.: - Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje po opuszczeniu programu – min. 22% grupy docelowej, -Liczba os.zagrożonych ubóstwem lub wykluczeniem społecznym, poszukujących pracy po opuszczeniu programu – min.56% gr.docel. Objective: An increased level of social and occupational activity among mines of 56 %, with GD and increased capacity to take up employment among 70 people at risk of poverty or social exclusion, residing in the Mazowieckie province, with a comprehensive support in the form of active inclusion services in proj.01.VI.2017-31.V.2018. The target group is set at 70 persons (42 K and 28 M) at risk of poverty or exclusion. Moreover: — 90 % GD is to be excluded and min.10 % GD with large or moderate disability. — min.60 % GD will represent K. Projajowaer to the GD from the Łaze.mazowan area. Within the framework of the pro.j.j.d.d.: 1.The identification of the individual needs and potential of the UP; 2.psychologiccounselling and psychosocial counselling, 3.Job placement and counselling; 4.Exercise of competences and social skills for disabled SPs 5.Training, 6.Professional qualifications. The results of the project include: — The number of people at risk of poverty or social exclusion, who have obtained a qualification after leaving the programme — min. 22 % of the target group, -Number of people at risk of poverty or social exclusion, looking for a job after leaving the programme — 56 % of the target population.
