Increasing access to care and social services in Kujawsko-Pomorskie province of Kujawsko-Pomorskie Province for 30 independent persons including older people and with disabilities in the period from 01.10.2020 to 30.09.2022 It is a partnership project of the Society of Development Society of the debtor — Lider and the Municipality of the debtor — Partner. The project provides for the implementation of comprehensive social services in the local environment, including: — Creation of a support point that will provide information, training and legal advice services, geriatrics and a psychologist for dependent persons and their carers, to improve the quality of care services for 60 participants (30 non-dependent and 30 executives). A consultant supporting the participants and the environment; — Social work with participants; — Provision of care services for dependent persons at the place of residence as a substitute for the care givers; — Provision of rehabilitation services for dependent persons. The services will be carried out by the trained song-/-s of the project of 5 carers. All activities will be carried out free of charge. Without the additional support from the European Union, the planned activities could not be realised. (English)

    Zwiększenie dostępu do usług opiekuńczych i społecznych świadczonych w Gminie Płużnica woj. kujawsko - pomorskie dla 30 niesamodzielnych osób w tym seniorów i z niepełnosprawnościami w okresie od 01.10.2020 do 30.09.2022r. Jest to projekt partnerski Towarzystwa RozwojuGminy Płużnica - Lider i Gminy Płużnica - Partner. Projekt przewiduje realizację kompleksowych usług społecznych realizowanych w środowisku lokalnym w tym: - Utworzenie Punktu wsparcia, który świadczyć będzie usługi informacyjne, szkoleniowe i doradcze prawne, lekarza geriatry i psychologa dla osób niesamodzielnych i ich opiekunów faktycznych służące poprawie jakości usług opiekuńczych dla 60 uczestników ( 30 osób niesamodzielnych i 30 opiekunów faktycznych). W Punkcie zatrudniony będzie konsultant wspierający uczestników i otoczenie;- Praca socjalna z uczestnikami; - Świadczenie usług opiekuńczych dla osób niesamodzielnych w miejscu zamieszkania w zastępstwie za opiekunów faktycznych;- Świadczenie usług rehabilitacyjnych dla osób niesamodzielnych. Usługi będą realizowane przez przeszkolonych/-ne w ramach projektu 5 opiekunów/-ek. Wszystkie działania realizowane w projekcie będą nieodpłatne. Bez dodatkowego wsparcia z Unii Europejskiej zaplanowanych działań nie można by było zrealizować. (Polish)
