Celem projektu jest zwiększenie wśród 300 (200K/100M) mieszkańców woj. łódzkiego, powiatu łódzki wsch.i gmin słabo zaludnionych, w wieku aktywności zawodowej pow. 45 r.ż. świadomości w zakresie występowania u nich wysokiego ryzyka zachorowania na cukrzycę, stanu przedcukrzycowego lub cukrzycy oraz poziomu wiedzy nt. sposobu redukcji czynników ryzyka rozwoju choroby poprzez ich udział w programie wczesnego wykrywania i profilatyki cukrzycy typu II w okresie 01.09.2018 – 30.06.2020 Projekt poprzez realizację programu wczesnego wykrywania i profilaktyki cukrzycy zrealizuje w 4 % założenia niniejszego konkursu. Założenia projektu przedstawione we wniosku o dofinansowanie są zgodne z założeniami regionalnego programu zdrowotnego (RPZ) załączonego do regulaminu konkursu. Zamierzamy zrealizować cel poprzez: 1. Działania informacyjno-promocyjne 2. Przyjmowanie zgłoszeń i kwalifikację do Programu 3. Etap badań przesiewowych 4. Etap działań informacyjno-edukacyjnych (Polish)

The aim of the project is to increase the population of Łódź Province, Łódzkie district of Białystok and sparsely populated municipalities to the age of 300 (200K/100M). 45. awareness of the prevalence of high risk diabetes, the state of diabetes and diabetes and the level of knowledge on how to reduce the risk factors of the development of the disease through their participation in a Type II early detection and learning programme for diabetes Type II period 01.09.2018-30.06.2020. the project will achieve 4 % of the design of this competition by implementing a diabetes early detection and prevention programme. The project’s assumptions in the application for funding are in line with the objectives of the Regional Health Programme (RPZ) attached to the competition rules. We intend to achieve the objective by: 1. Information and publicity 2. Acceptance of notifications and qualification for Programme 3. Screening phase 4. Information and education phase (English)
