The project is aimed at people from rural areas experiencing multiple social exclusion, both unemployed, inactive, disabled, farmers and depending on the diagnosis of the needs of people at risk of poverty or social exclusion. The main objective of the project is to counteract social exclusion through social, health and professional activation of people benefiting from social assistance. Each participant of the project will be diagnosed by an integration path specialist, psychologist and professional advisor in terms of needs and opportunities. Next, depending on the diagnosis, the participant will be able to use the Active Integration Services of a social nature (individual and group meetings with a psychologist, the possibility of using a lawyer’s advice, the basics of computer use under the supervision of a computer scientist — a digital exclusion specialist, social competency workshops and parental competence workshops (both in stationary and away form), support group dependent on the diagnosis of individual needs and Active integration services of a health nature (possibility of using physiotherapist services in order to eliminate or alleviate health barriers that hinder functioning or lead to distance from the labour market). Project support according to the diagnosis will be adapted in a flexible manner to the needs of each participant. In addition, participants will be able to take advantage of the Active Vocational Integration Services, where each person will receive the support of a professional advisor at group meetings as well as an Employee Trainer. Supported and in accordance with the Individual Action Plan (IPD), it will be able to benefit from trainings, traineeships preceded by research. Our comprehensive support is aimed at increasing the life competences of the participants, regaining economic stability, sense of security, confidence in their own strengths and opportunities (English) | |||

Projekt jest skierowany do osób z terenów wiejskich doświadczających wielokrotnego wykluczenia społecznego, zarówno osób bezrobotnych, nieaktywnych zawodowo, niepełnosprawnych, rolników oraz zależnie od diagnozy potrzeb otoczenia osób zagrożonych ubóstwem lub wykluczeniem społecznym. Celem głównym projektu jest przeciwdziałanie wykluczeniu społecznemu poprzez aktywizację społeczną, zdrowotną i zawodową osób korzystających z pomocy społecznej. Każdy uczestnik projektu zostanie zdiagnozowany przez specjalistę do spraw ścieżki integracji, psychologa oraz doradcę zawodowego pod kątem potrzeb i możliwości. Następnie w zależności od diagnozy uczestnik będzie mógł skorzystać z Usług aktywnej integracji o charakterze społecznym (spotkania indywidualne i grupowe z psychologiem, możliwość skorzystania z porad prawnika, podstawy obsługi komputera pod okiem informatyka - specjalista ds. wykluczenia cyfrowego, warsztatów kompetencji społecznych oraz warsztatów kompetencji rodzicielskich ( zarówno w formie stacjonarnej jak i wyjazdowej), grupy wsparcia uzależnionej od diagnozy indywidualnych potrzeb oraz Usług aktywnej integracji o charakterze zdrowotnym ( możliwość skorzystania z usług fizjoterapeuty w celu wyeliminowania lub złagodzenia barier zdrowotnych utrudniających funkcjonowanie lub powodujących oddalenie od rynku pracy). Wsparcie w ramach projektu zgodnie z przeprowadzoną diagnozą będzie dostosowane w sposób elastyczny do potrzeb każdego uczestnika. Ponadto uczestnicy będą mogli skorzystać z Usług aktywnej integracji zawodowej, gdzie każda osoba otrzyma wsparcie doradcy zawodowego na spotkaniach grupowych jak i Trenera Zatrud. Wspieranego oraz zgodnie z Indywidualnym Planem Działania ( IPD) będzie mogła skorzystać ze szkoleń, staży zawodowych poprzedzonych badaniami.
