The project was implemented in the period 01.05.2022-31.05.2023 by Habilis sp. z o.o. in partnership with the medicinal entity “REMED” Renata Bienias and NZOZ Health Center in Graczach Jacek Ciepluch i wspólnicy jawna. The project is aimed at people at risk of poverty or social exclusion (old people), the environment of people at risk of poverty or social exclusion (actual caregivers) and staff of services providing services in the local community living in the territory of the Polish Voivodeship, in particular the Oleskie district and the city of Kluczbork. Support will be provided to 8 dependent elderly people (6K and 2M), to whom support will be provided in the form of a comprehensive range of health services, including long-term.medical home care in replacement for actual caregivers, together with psychological and rehab support, and increasing access to services through the provision of necessary transport. The project will also strengthen the skills and competences of 5 caregivers of real dependants (4K,1M) through the use of psychological support and individual training in medical care. The project aims to develop de-institutionalised forms of medical care for the elderly, including those with disabilities, by supporting the provision of medical services in the home environment of patients. Projekt realizowany w okresie 01.05.2022-31.05.2023 przez Habilis sp. z o.o. w partnerstwie z podmiotem leczniczym "REMED" Renata Bienias oraz NZOZ Ośrodek Zdrowia w Graczach Jacek Ciepluch i wspólnicy spółka jawna. Projekt skierowany jest do osób zagrożonych ubóstwem lub wykluczeniem społecznym (os. w podeszłym wieku), otoczenia osób zagrożonych ubóstwem lub wykluczeniem społecznym (opiekunowie faktyczni) oraz personelu służb świadczącego usługi w społeczności lokalnej zamieszkujących teren woj.opolskiego, w szczególności powiatu oleskiego i miasta Kluczbork. Wsparciem zostanie objętych 8 osób niesamodzielnych w podeszłym wieku (6K i 2M), którym zapewnione zostanie wsparcie w formie kompleksowego zakresu usług zdrowotnych, w tym długoterm.medycznej opieki domowej w zast. za opiekunów faktycznych wraz z wsparciem psychologicznym i rehablitacyjnym oraz zwiększenie dostępu do usług poprzez zapewnienie niezbędnego transportu. Projekt wzmocni również umiejętności i kompetencje 5 opiekunów faktycznych osób niesamodzielnych (4K,1M) poprzez korzystanie z wsparcia psychologicznego i indywidualne szkolenia z zakr.opieki medycznej. Projekt ma na celu rozwój zdeinstytucjonalizowanych form opieki medycznej nad osobami starszymi, w tym z niepełnosprawnościami poprzez wsparcie świadczenia usług medycznych w środowisku domowym pacjentów.
