The Opolskie project for the elderly — development of health services in medical care as a substitute for actual caregivers and support for real carers in the field of holistic medical care, will be implemented within the framework of Measure 8.1 Access to high-quality health and social services in the field of health services — care for the elderly, including with disabilities, Axis VIII Social Integration ROP WO 2014-2020. The following needs have been identified: Provision of medical care as a substitute for actual caregivers (employment of 3 nurses and 2 physiotherapists); Implementation at local level of solutions for coordinating the delivery of medical services; Improving the competences and qualifications of health service staff for forms of de-institutionalised care. Results generated during the project: The medical service provided at the level of the local community will contribute to enabling independent living in the local environment, does not result in isolation from the family and the local community (allowing the maintenance of family and neighbourhood ties) — is individualised — the organizational requirements related to its granting do not take precedence over the individual needs of the user. The activities planned in the project are consistent with the assumptions of the SSD Program, as part of Package IV Golden Autumn. Number of people supported: 40 elderly people, including those with disabilities, who need support in their daily functioning and require the inclusion of de-institutionalised forms of medical care in their daily functioning. 40 % of the participants will be patients after leaving the hospital or ZOL to continue treatment and post-hospital care, and 35 % of people with disabilities. Planned period of project implementation: 01.05.2022-31.05.2023. Total cost of the project: 462 500,00 zł. Requested amount of support: 437 062.50 PLN (94.50 %). (English) | |||

Projekt Opolskie dla osób starszych - rozwój usług zdrowotnych w opiece medycznej w zastępstwie za opiekunów faktycznych oraz wsparcie dla opiekunów faktycznych w zakresie holistycznej opieki medycznej, realizowany będzie w ramach działania 8.1 Dostęp do wysokiej jakości usług zdrowotnych i społecznych w zakresie usług zdrowotnych - opieki nad osobami starszymi, w tym z niepełnosprawnościami, Osi VIII Integracja społeczna RPO WO 2014-2020. Zidentyfikowano następujące potrzeby: Zapewnienie opieki medycznej w zastępstwie za opiekunów faktycznych (zatrudnienie 3 pielęgniarek oraz 2 fizjoterapeutów); Wdrożenie na poziomie lokalnym rozwiązań z zakresu koordynacji realizacji usług medycznych; Podnoszenie kompetencji i kwalifikacji personelu służb świadczących usługi zdrowotne dotyczące form opieki zdeinstytucjonalizowanej. Rezultaty wygenerowane w trakcie trwania projektu: Usługa medyczna świadczona na poziomie lokalnej społeczności przyczyni się do umożliwienia niezależnego życia w środowisku lokalnym, nie powoduje odizolowania od rodziny i środowiska lokalnego (pozwoli na podtrzymanie więzi rodzinnych i sąsiedzkich) - jest zindywidualizowana - wymagania organizacyjne związane z jej udzieleniem nie mają pierwszeństwa nad indywidualnymi potrzebami osoby z niej korzystającej. Działania zaplanowane w projekcie są spójne z założeniami Programu SSD, w ramach Pakietu IV Złota jesień. Ilość osób objętych wsparciem: 40 osób starszych, w tym z niepełnosprawnościami, potrzebujących wsparcia w codziennym funkcjonowania i wymagające włączenia zdeinstytucjonalizowanych form opieki medycznej w codziennym funkcjonowaniu. 40 % uczestników projektu stanowić będą pacjenci po opuszczeniu szpitala lub ZOL w celu kontynuacji leczenia i opieki po hospitalizacji, oraz 35 % osób niepełnosprawnych. Planowany okres realizacji projektu: 01.05.2022 r. – 31.05.2023 r. Całkowity koszt projektu: 462 500,00 zł. Wnioskowana kwota wsparcia: 437 062,50 zł (94,50%). (Polish)
