The aim of the project is to: Increasing the social, professional and educational activity of 60 (39K/21M) people at risk of poverty or social exclusion (in particular due to long-term unemployment, disability, age-related dependency, young people from families at risk of social exclusion, people leaving prisons, people who are homeless) from rural areas of the Lubuskie Voivodeship in the period from 01.03.2022 to 31.05.2023 based on a reintegration path, created individually for each person. Under the project, the supported groups will be marginalised. Each UP will receive comprehensive support, among others, in the field of diagnosis of needs and IEC, social competence workshops, support of a social worker, family assistant, legal and financial counselling, TOR, vocational training, internships, employment placements and associate psychologists, Support addressed to UP will assume mandatory individual social diagnosis for each UP and adaptation to its results. The employment efficiency ratio will be 12 % for SA and 25 % for other groups. 34 % for ON and 34 % of other groups (English)

    Celem projektu jest: Zwiększenie aktywności społecznej, zawodowej i edukacyjnej 60 (39K/21M) osób zagrożonych ubóstwem lub wykluczeniem społecznym (w szczególności z powodu długotrwałego bezrobocia, niepełnosprawności, niesamodzielności ze względu na wiek, młodzieży z rodzin zagrożonych wykluczeniem społecznym, os. opuszczające zakłady karne, os. bezdomne) z obszarów wiejskich województwa lubuskiego w okresie od 01.03.2022 do 31.05.2023 w oparciu o ścieżkę reintegracji, stworzoną indywidualnie dla każdej osoby. Wramach projektu objęte wsparciem zostaną grupy marginalizowane. Każdy UP otrzyma kompleksowe wsparcie m.in. w zakresie zdiagnozowania potrzeb i IŚR, warsztatów kompetencji społecznych, wsparcia pracownika socjalnego,asystenta rodziny, poradnictwa prawnego i finansowego, TZW, szkoleń zawodowych, staży, pośrednictwa pracy i wsp. psychologa, Wsparcie kierowane do UP będzie zakładać obowiązkowe przeprowadzenie indywidualnej diagnozy społ.-zaw. dla każdego UP i dostos. wsp.do jej wyników. Wskaźnik efektywności zatrudnieniowej wyniesie 12% dla ON i 25% dla pozostałych grup;wsk. ef. społ. 34% dla ON i 34% pozostałych grup (Polish)
