The main objective of the project is to increase the number of wasps covered by social services in the above-mentioned beginning. (Item) at 40 (28K+ 12M) GIPs at risk of poverty/exclusion of society, aimed at satisfying the growing needs of the above-mentioned persons resulting from reliance, inter alia through the provision of care services and specialists. care services at the place of the above-mentioned persons, provision of services for the delivery of meals for the GIP to their premises, the establishment of a rental of rehabilitation and care equipment and support of the GIP environment, i.e. 40 OF (32K+ 8M) above OPW — improving the mental condition of OF through psychological support of the aforementioned and increasing the knowledge of the above mentioned above. Of, among others, moving around various support schemes, the use of which is necessary for the provision of high-quality care in the area of implementation of the project: 01.07.2022-31.08.2023. Project. skier. to GD from WŁ (physical person, learner/worker/in the above-mentioned beginning with ILO in accordance with the CC) i.e.: *40 GIP (28K+ 12M) i.e. a person at risk of poverty/exclusion of society regardless of age and status of the group which, due to the state of health or disability, require care or support in connection with the impossibility of self-execution. at least 1 of the basic activities of everyday life *enclosure of the GIP, i.e. 40 OF (32K+ 8M) i.e. adults caring for GIPs (not carers of profession. and not receiving compensation from the title of custody of the GIP). Tasks in the project: 1.develops. individual support paths in project. 2.care services at the place of the castle. 3.Specialised care services in the place of the castle. 4.services for delivering meals 5.establishment of rehabilitation and care equipment rental 6. support OF GIP (psychological counselling, individual counselling, counselling, among others in moving around various support systems). The effects and results of the project: +L. people at risk of poverty/exclusion of the social services provided in the general interest:40 +L. o. from the environment of poverty/exclusion of the community supported in the threshold of:40 +L. persons covered by the services from the delivery of meals: 28 +L. supported in the progr. places of provision of social services. Celem głównym proj. jest zwiększenie liczby os objętych usługami społ. w pow. opocz. (WŁ) o 40 (28K+12M) OPW zagroż. ubóstwem/ wyklucz. społ., mającymi służyć zaspokojeniu indywid. rosnących potrzeb ww. os wynikających z niesamodzielności m.in poprzez realiz. usług opiekuńczych i specjalist. usług opiek. w miejscu zam. ww. os., świadczenie usług dowożenia posiłków dla OPW do miejsca ich zam., utworzenie wypożyczalni sprzętu rehabilit. i opiekuńcz. oraz wsparcie otoczenia OPW tj. 40 OF (32K+8M) ww. OPW - poprawa kondycji psychicznej OF poprzez wsparcie psychologiczne ww. i zwiększenie wiedzy ww. OF m.in. z zakr. poruszania się po rożnych systemach wsparcia z których korzystanie jest niezbędne dla sprawowania wysokiej jakości opieki w okr. realiz. proj.: 01.07.2022-31.08.2023. Zadania w proj: 1.oprac. indywid. ścieżek wsparcia w proj. 2.usługi opiekuńcze w miejscu zam. 3.specjalistyczne usługi opiekuńcze w miejscu zam. 4.usługi dowożenia posiłków 5.utworzenie wypożyczalni sprzętu rehabilit. i opiekuńczego 6. wsparcie OF OPW (poradnictwo psychologiczne, indywid. poradn. m.in w zakr. poruszania się po rożnych systemach wsparcia).
