The main objective of the project (P) is to support de-institutionalisation and quality and access to medical and care services to meet the growing needs resulting from social dependency for 465os(242K;223M) in need of support in everyday functioning, including the elderly or with disabilities who learn/work or reside in reason. Links 705os(368K 337M) will be supported under each activity. The number of people in need of support in the daily functioning of the 465os project(242K;223M). Number of people in the environment of people at risk of poverty and social exclusion supported by project 240os(126K;114M). Number of persons with disabilities participating in project 53os(28K25M). The goal will be achieved through the establishment and functioning of the Kutnów Centre for Environmental Services (KCUŚ). Within the framework of the KCUŚ there will be a Daily Residence House (DDP1) in Strzelce, DDP2 in Żychlin, offering care services in the place of the castle, assistants, 2 rentals of rehab equipment — care, access to services: legal, Psychologist,Information and Consultancy. KCUŚ also offers support to support the needs of support in the function (actual caregivers, families) 240os(126K;114M) P will contribute to achieving the objective of ROP WŁ 2014-2020 by increasing the availability of social services in the region through the creation of the KCUŚ by reducing poverty and exclusion of people’s needs in the region. Celem gł projektu(P) jest wsparcie deinstytucjonalizacji oraz popr jakości i dostępu do usł medyczno-opiekuńczych służących zaspokojeniu rosnących potrzeb wynikających z niesamodzielności–w wymiarze społecznym dla 465os(242K;223M) potrzebujących wsparcia w codziennym funkcjonowaniu, w tym os starszych lub z niepełnosprawnościami które uczą się/pracują lub zamieszkują w rozum.przep KC na obsz pow. kutnowskiego w woj. łódz oraz 240os(126K;114M) ich otoczenia w okr 01.10.2021-30.06.2023. Wsparciem w ramach poszczeg działań zostanie objęte łącz 705os(368K 337M). liczba osób potrzebujących wsparcia w codziennym funkcjonowaniu biorących udział w projekcie 465os(242K;223M). Liczba osób z otoczenia osób zagrożonych ubóstwem i wykluczeniem społecznym objęta wsparciem w projekcie 240os(126K;114M). Liczba osób z niepełnosprawnościami biorących udział w projekcie 53os(28K25M). Os. z niepełnosprawnościami są os. potrzeb. wsparcia w codziennym funkcjonowaniu Cel zostanie zrealizowany poprzez utworzenie i funkcjonowanie Kutnowskiego Centrum Usług Środowiskowych(KCUŚ). W ramach KCUŚ powstanie Dzienny Dom Pobytu (DDP1) w gm. Strzelce, DDP2 w Żychlinie, oferowane będą usł opiekuńcze w miejscu zam, asystenckie, 2 wypożyczalnie sprzętu rehab - opiek, dostep do usług: prawnych, psycholog ,inform i doradczych. KCUŚ oferuje też dział wspier otoczenie os. potrzeb wsparcia w codz funkcj (opiekunów faktycznych, rodzin) 240os(126K;114M) P przyczyni się do osiągnięcia wł celu szczeg RPO WŁ 2014-2020 zwiększając dostępność do realizowanych w regionie usług społ. poprzez utworzenie KCUŚ ograniczając ubóstwo i wykluczenie społ odp na zdiag potrzeby w regionie (Polish)
