Le projet vise à proposer une prise en charge globale de jour de type communautaire (12 semaines) permettant une coordination du dispositif médico-psycho-social autour du jeune et de sa famille. Il s’adresse aux adolescents et jeunes adultes (15 à 23 ans) présentant des comportements d’assuétude aux substances psychoactives en lien avec d’autres difficultés (décrochage scolaire, conflits familiaux, incivilités, violence,…) qui engendrent des répercussions importantes ou non sur leur santé physique et psychique ainsi que sur leur processus scolaire et leur insertion socioprofessionnelle. The aim of the project is to provide comprehensive care for community-type days (12 weeks) allowing coordination of the medical-psycho-social system around the young person and his/her family. It is aimed at adolescents and young adults (15-23 years old) who have behaviours of assumption of psychoactive substances in connection with other difficulties (early school leaving, family conflicts, incivilities, violence, etc.) which have or may not have a significant impact on their physical and mental health as well as on their school process and their socio-professional integration. Het doel van het project is uitgebreide zorg te bieden voor gemeenschapsdagen (12 weken) waardoor het medisch-psychosociale systeem rond de jongere en zijn/haar gezin kan worden gecoördineerd. Het is bedoeld voor adolescenten en jongvolwassenen (15-23 jaar) die gedrag vertonen waarbij psychoactieve stoffen worden verondersteld in verband met andere problemen (vroegtijdig schoolverlaten, familieconflicten, incivielen, geweld, enz.) die al dan niet aanzienlijke gevolgen hebben voor hun lichamelijke en geestelijke gezondheid, hun schoolproces en hun sociaal-professionele integratie.
