С проектното си предложение община Съединение подкрепя Център, създаден по процедура „Помощ в дома“, за почасово предоставяне на услуги за социално включване в общността или в домашна среда, в т.ч. подкрепящи и интегрирани социални и здравни услуги на общинско ниво, според специфичните потребности на хора с увреждания и техните семейства и лица над 65 г. с ограничения или в невъзможност за самообслужване, въз основа на индивидуална социална оценка. В настоящия проект като потребители ще бъдат включени и лицата ползвали услуги по процедура „Нови алтернативи” след приключване на дейностите през март 2016 г. С проекта ще се подкрепят и семействата, на лицата от целевата група, като им се даде възможност за връщане на пазара на труда.За качествено и ефективно организиране, координиране, изпълнение, контрол и отчитане на цялостната дейност на проекта за постигане на заложените цели ще бъде сформиран екип за управление. Ще бъде осигурена публичност и разпространена информация относно конкретните цели и резултати от проекта и финансовия принос на ЕС. По проекта ще бъде подбран и нает персонал в центъра, необходим за цялостното му функциониране, управление и предоставяне на услуги - социален асистент, домашен помощник/санитар, личен асистент, управител на центъра, социален работник, шофьор, както и персонал за предоставяне на специализирани услуги в домашни условия и в общността - медицинска сестра, рехабилитатор, психолог. Персоналът за предоставяне на услуги ще бъде обучен и ще му се предостави психологическа подкрепа и/или консултиране. На потребителите на услуги ще се окаже мотивационна и/или психологическа подкрепа, според тяхната индивидуална потребност. Ще се закупи специализиран транспорт за хора с увреждания за транспортиране на лицата от целевата група за предоставяне на специализирани услуги в центъра. За функционирането на центъра по проекта ще бъде извършен ремонт и ще се закупи оборудване. (Bulgarian)

With its project proposal, municipality of Saedineje supports a Center established under the procedure “Help in the home” for hourly provision of services for social inclusion in the community or in a home environment, including supporting and integrated social and health services at municipal level, according to the specific needs of people with disabilities and their families and persons over 65 years old, with restrictions or inability to self-service, based on an individual social assessment. In this project will be included as users the beneficiaries of the New Alternatives procedure after the completion of the activities in March 2016. The project will also support the families of the target group by enabling them to return to the labour market. For quality and effective organisation, coordination, implementation, control and reporting of the overall project activity to achieve the objectives set will be set up. Publicity and dissemination of information on the specific objectives and results of the project and the EU financial contribution will be provided. The project will select and employ staff in the centre, necessary for its overall functioning, management and provision of services — social assistant, home assistant/healthcare, personal assistant, manager of the centre, social worker, driver, as well as staff to provide specialised services at home and in the community — nurse, rehabilitator, psychologist. The service staff will be trained and provided with psychological support and/or counselling. Service users will be motivated and/or psychologically supported according to their individual needs. Specialised transport for people with disabilities will be purchased to transport the targeted persons to provide specialised services in the centre. For the operation of the centre, the project will be repaired and equipment will be purchased. (English)

Avec sa proposition de projet, la municipalité de Saedineje soutient un centre créé dans le cadre de la procédure «Aide à la maison» pour la prestation horaire de services d’inclusion sociale dans la communauté ou dans un environnement familial, y compris des services sociaux et de santé d’appui et intégrés au niveau municipal, en fonction des besoins spécifiques des personnes handicapées et de leur famille et des personnes âgées de plus de 65 ans, avec des restrictions ou une incapacité de libre-service, sur la base d’une évaluation sociale individuelle. Dans ce projet seront inclus en tant qu’utilisateurs les bénéficiaires de la procédure «Nouvelles alternatives» après l’achèvement des activités en mars 2016. Le projet soutiendra également les familles du groupe cible en leur permettant de retourner sur le marché du travail. La publicité et la diffusion d’informations sur les objectifs spécifiques et les résultats du projet ainsi que sur la contribution financière de l’UE seront fournies. Le projet sélectionnera et emploiera du personnel nécessaire à son fonctionnement global, à la gestion et à la prestation des services — assistant social, assistant à domicile/soins de santé, assistant personnel, gestionnaire du centre, travailleur social, chauffeur, ainsi que du personnel chargé de fournir des services spécialisés à domicile et dans la communauté — infirmière, rééducation, psychologue. Le personnel du service sera formé et bénéficiera d’un soutien psychologique et/ou d’un conseil. Les utilisateurs de services seront motivés et/ou soutenus psychologiquement en fonction de leurs besoins individuels. Des transports spécialisés pour les personnes handicapées seront achetés pour transporter les personnes ciblées afin de fournir des services spécialisés dans le centre. Pour l’exploitation du centre, le projet sera réparé et l’équipement sera acheté. (French)

Mit ihrem Projektvorschlag unterstützt die Gemeinde Saedineje ein nach dem Verfahren „Help in the home“ eingerichtetes Zentrum für die stundenlange Erbringung von Dienstleistungen für die soziale Inklusion in der Gemeinschaft oder in einem häuslichen Umfeld, einschließlich Unterstützung und integrierter Sozial- und Gesundheitsdienste auf kommunaler Ebene, entsprechend den spezifischen Bedürfnissen von Menschen mit Behinderungen und ihren Familien und Personen über 65 Jahre, mit Einschränkungen oder Unfähigkeit der Selbstbedienung, basierend auf einer individuellen sozialen Bewertung. In dieses Projekt werden die Nutzer des Verfahrens „Neue Alternativen“ nach Abschluss der Aktivitäten im März 2016 einbezogen. Das Projekt wird auch die Familien der Zielgruppe unterstützen, indem sie sie in die Lage versetzen, auf den Arbeitsmarkt zurückzukehren. Für eine qualitativ hochwertige und wirksame Organisation, Koordinierung, Durchführung, Kontrolle und Berichterstattung über die gesamte Projekttätigkeit zur Erreichung der gesetzten Ziele werden sie eingerichtet. Die Öffentlichkeit und die Verbreitung von Informationen über die spezifischen Ziele und Ergebnisse des Projekts und den finanziellen Beitrag der EU werden bereitgestellt. Im Rahmen des Projekts werden Mitarbeiter des Zentrums ausgewählt und eingestellt, die für die allgemeine Funktionsfähigkeit, die Verwaltung und die Erbringung von Dienstleistungen erforderlich sind – Sozialassistent, Heimassistent/Gesundheitsassistent, persönlicher Assistent, Leiter des Zentrums, Sozialarbeiter, Fahrer sowie Mitarbeiter, die zu Hause und in der Gemeinschaft spezialisierte Dienstleistungen erbringen – Krankenschwester, Rehabilitator und Psychologe. Das Servicepersonal wird geschult und mit psychologischer Unterstützung und/oder Beratung versorgt. Die Servicenutzer werden entsprechend ihrer individuellen Bedürfnisse motiviert und/oder psychologisch unterstützt. Spezialisierte Beförderungen für Menschen mit Behinderungen werden gekauft, um die Zielpersonen zu befördern, um spezialisierte Dienstleistungen im Zentrum zu erbringen. Für den Betrieb des Zentrums wird das Projekt repariert und Ausrüstung gekauft. (German)

Met haar projectvoorstel ondersteunt de gemeente Saedineje een centrum dat is opgericht volgens de procedure „Help in de woning” voor het verlenen van diensten voor sociale inclusie in de gemeenschap of in een thuisomgeving, met inbegrip van ondersteunende en geïntegreerde sociale en gezondheidsdiensten op gemeentelijk niveau, afhankelijk van de specifieke behoeften van mensen met een handicap en hun gezinnen en personen ouder dan 65 jaar, met beperkingen of onvermogen tot zelfbediening, op basis van een individuele sociale beoordeling. In dit project zullen als gebruikers de begunstigden van de New Alternatives-procedure worden opgenomen na de voltooiing van de activiteiten in maart 2016. Het project zal ook de families van de doelgroep ondersteunen door hen in staat te stellen terug te keren naar de arbeidsmarkt. Voor kwaliteit en effectieve organisatie, coördinatie, uitvoering, controle en rapportage van de algemene projectactiviteit om de gestelde doelstellingen te bereiken zal worden opgezet. Er zal worden gezorgd voor publiciteit en verspreiding van informatie over de specifieke doelstellingen en resultaten van het project en de financiële bijdrage van de EU. Het project zal personeel selecteren en in dienst nemen in het centrum, dat nodig is voor de algemene werking, het beheer en de dienstverlening — sociaal assistent, thuisassistent/gezondheidszorg, persoonlijke assistent, manager van het centrum, sociaal werker, chauffeur, alsmede personeel dat gespecialiseerde diensten thuis en in de gemeenschap verleent — verpleegkundige, revalidateur, psycholoog. Het personeel van de dienst wordt opgeleid en voorzien van psychologische ondersteuning en/of begeleiding. Servicegebruikers worden gemotiveerd en/of psychologisch ondersteund op basis van hun individuele behoeften. Gespecialiseerd vervoer voor personen met een handicap zal worden gekocht om de beoogde personen te vervoeren om gespecialiseerde diensten in het centrum te verlenen. Voor de exploitatie van het centrum wordt het project gerepareerd en wordt apparatuur aangekocht. (Dutch)

Con la sua proposta di progetto, il comune di Saedineje sostiene un Centro istituito secondo la procedura "Aiuto in casa" per la fornitura oraria di servizi per l'inclusione sociale nella comunità o in un ambiente domestico, compresi servizi sociali e sanitari integrati a livello comunale, in base alle esigenze specifiche delle persone con disabilità e delle loro famiglie e delle persone di età superiore ai 65 anni, con restrizioni o incapacità al self-service, sulla base di una valutazione sociale individuale. In questo progetto saranno inclusi come utenti i beneficiari della procedura New Alternatives dopo il completamento delle attività nel marzo 2016. Il progetto sosterrà anche le famiglie del gruppo target consentendo loro di tornare sul mercato del lavoro. Per un'organizzazione, un coordinamento, un coordinamento, l'attuazione, il controllo e la rendicontazione dell'attività complessiva del progetto per raggiungere gli obiettivi stabiliti, saranno istituiti per qualità ed efficacia nell'organizzazione, coordinamento, attuazione, controllo e rendicontazione. I trasporti specializzati per le persone con disabilità saranno acquistati per trasportare le persone destinatarie per fornire servizi specializzati nel centro. Per il funzionamento del centro, il progetto sarà riparato e le attrezzature saranno acquistate. (Italian)

Con su propuesta de proyecto, el municipio de Saedineje apoya un Centro establecido bajo el procedimiento «Ayuda en el hogar» para la prestación de servicios por hora para la inclusión social en la comunidad o en un entorno domiciliario, incluyendo el apoyo e integrado servicios sociales y de salud a nivel municipal, de acuerdo con las necesidades específicas de las personas con discapacidad y sus familias y personas mayores de 65 años, con restricciones o incapacidad para el autoservicio, basado en una evaluación social individual. En este proyecto se incluirá como usuarios a los beneficiarios del procedimiento de Nuevas Alternativas tras la finalización de las actividades en marzo de 2016. El proyecto también apoyará a las familias del grupo destinatario permitiéndoles regresar al mercado laboral. Para una organización de calidad y eficaz, se establecerá la coordinación, la ejecución, el control y la presentación de informes de la actividad global del proyecto para alcanzar los objetivos fijados. Se dará publicidad y difusión de información sobre los objetivos y resultados específicos del proyecto y la contribución financiera de la UE. El proyecto seleccionará y empleará personal en el centro, necesario para su funcionamiento general, gestión y prestación de servicios -asistente social, ayudante del hogar/asistencia sanitaria, asistente personal, gerente del centro, trabajador social, conductor, así como personal para prestar servicios especializados en el hogar y en la comunidad — enfermera, rehabilitadora, psicólogo. El personal del servicio recibirá formación y se proporcionará apoyo psicológico o asesoramiento. Los usuarios del servicio serán motivados o apoyados psicológicamente de acuerdo a sus necesidades individuales. Se adquirirá un transporte especializado para las personas con discapacidad para transportar a las personas a las que se dirigen para prestar servicios especializados en el centro. Para el funcionamiento del centro, se reparará el proyecto y se adquirirán equipos. (Spanish)
