The aim of the project is to provide integral assistance (social care and care) at home for disabled people of working age and elderly and advisory assistance to their family members. During the project we will carry out the following activities: training and professional development of team personnel; provision of integral assistance to disabled people of working age and the elderly; counselling for family members caring for elderly relatives with disabilities; informal aid promotion event for community members. Integral assistance per person will be provided up to 4.5 hours per day, up to 5 days per week. It will be provided by 3 mobile teams. 2 cars, technical equipment, nursing equipment and rehabilitation measures that are needed to provide integral support, adults with disabilities and elderly people will be purchased. The project will provide individual and group advice to 16 family members.The project will result in staff gaining more professional experience (30 persons), providing integrated (social care and care) home services (70 recipients), meeting the vital needs of disabled and elderly people, providing advice to family members on issues of concern to them (16 persons), encouraging community and volunteers involvement in informal assistance (19). Projekto tikslas- suteikti integralią pagalbą (socialinę globą ir slaugą) namuose neįgaliems darbingo amžiaus ir senyvo amžiaus asmenims bei konsultacinę pagalbą jų šeimų nariams. Projekto metu vykdysime šias veiklas: komandų darbuotojų mokymas ir profesinės kompetencijos kėlimas; integralios pagalbos teikimas neįgaliems darbingo amžiaus ir senyvo amžiaus žmonėms; šeimos narių, prižiūrinčių senyvo amžiaus neįgalius artimuosius, konsultavimas; neformalios pagalbos skatinimo renginys, skirtas bendruomenės nariams. Integrali pagalba 1 asmeniui bus teikiama iki 4,5 val. per dieną, iki 5 dienų per savaitę. Ją teiks 3 mobilios komandos.Bus įsigyti 2 automobiliai, techninė įranga, slaugos priemonės ir priemonės reabilitacijai , kurios reikalingos integraliai pagalbai teikti, suaugusiems asmenims su negalia ir senyvo amžiaus asmenims. Įgyvendinant projektą, bus teikiamos individualios ir grupinės konsultacijos 16 šeimos narių.Laukiamas projekto rezultatas –darbuotojai įgis daugiau profesinės patirties (30 asmenų), bus suteiktos integralios (socialinės globos ir slaugos) namuose paslaugos (70 gavėjų), patenkinti gyvybiškai svarbūs neįgalių ir senyvo amžiaus asmenų poreikiai, užtikrintas šeimos narių konsultavimas jiems rūpimais klausimais (16 asmenų), paskatinsime bendruomenės ir savanorių įsitraukimą į neformaliosios pagalbos teikimą (19 asmenų). Projektui viešinti numatomi straipsniai, lankstinukai, plakatai ir kt. (Lithuanian)
