With its project proposal, municipality of Saedineje supports a Center established under the procedure “Help in the home” for hourly provision of services for social inclusion in the community or in a home environment, including supporting and integrated social and health services at municipal level, according to the specific needs of people with disabilities and their families and persons over 65 years old, with restrictions or inability to self-service, based on an individual social assessment. In this project will be included as users the beneficiaries of the New Alternatives procedure after the completion of the activities in March 2016. The project will also support the families of the target group by enabling them to return to the labour market. For quality and effective organisation, coordination, implementation, control and reporting of the overall project activity to achieve the objectives set will be set up. Publicity and dissemination of information on the specific objectives and results of the project and the EU financial contribution will be provided. The project will select and employ staff in the centre, necessary for its overall functioning, management and provision of services — social assistant, home assistant/healthcare, personal assistant, manager of the centre, social worker, driver, as well as staff to provide specialised services at home and in the community — nurse, rehabilitator, psychologist. The service staff will be trained and provided with psychological support and/or counselling. Service users will be motivated and/or psychologically supported according to their individual needs. Specialised transport for people with disabilities will be purchased to transport the targeted persons to provide specialised services in the centre. С проектното си предложение община Съединение подкрепя Център, създаден по процедура „Помощ в дома“, за почасово предоставяне на услуги за социално включване в общността или в домашна среда, в т.ч. подкрепящи и интегрирани социални и здравни услуги на общинско ниво, според специфичните потребности на хора с увреждания и техните семейства и лица над 65 г. с ограничения или в невъзможност за самообслужване, въз основа на индивидуална социална оценка. В настоящия проект като потребители ще бъдат включени и лицата ползвали услуги по процедура „Нови алтернативи” след приключване на дейностите през март 2016 г. С проекта ще се подкрепят и семействата, на лицата от целевата група, като им се даде възможност за връщане на пазара на труда.За качествено и ефективно организиране, координиране, изпълнение, контрол и отчитане на цялостната дейност на проекта за постигане на заложените цели ще бъде сформиран екип за управление. Ще бъде осигурена публичност и разпространена информация относно конкретните цели и резултати от проекта и финансовия принос на ЕС. По проекта ще бъде подбран и нает персонал в центъра, необходим за цялостното му функциониране, управление и предоставяне на услуги - социален асистент, домашен помощник/санитар, личен асистент, управител на центъра, социален работник, шофьор, както и персонал за предоставяне на специализирани услуги в домашни условия и в общността - медицинска сестра, рехабилитатор, психолог. Персоналът за предоставяне на услуги ще бъде обучен и ще му се предостави психологическа подкрепа и/или консултиране. На потребителите на услуги ще се окаже мотивационна и/или психологическа подкрепа, според тяхната индивидуална потребност. Ще се закупи специализиран транспорт за хора с увреждания за транспортиране на лицата от целевата група за предоставяне на специализирани услуги в центъра. За функционирането на центъра по проекта ще бъде извършен ремонт и ще се закупи оборудване. (Bulgarian)
