The project “Patronage care for elderly and disabled people in Lovech Municipality” will be implemented under the procedure through direct grant award BG05M9OP001-2.040 “Patronage care for elderly and disabled people — Component 2” within Priority Axis 2 “Reducing poverty and promoting social inclusion” under OP HRD 2014-2020. The project provides for the provision of a new type of service on the territory of Lovech Municipality through the establishment of a network of health and social services in a home environment and is aimed at direct users — elderly people and persons with disabilities, including chronic diseases and permanent disabilities, as well as at the persons involved in the provision of care. The implementation of the project activities will help people living alone in small settlements and with the inability to take care of themselves, when patients are discharged from a hospital with the need for domestic medical care and people with disabilities. The provision of the patronage care service will provide visits to a social assistant to assist in everyday life and home visits by a qualified health care professional, which will ease the need to travel to a GP and to supply medicines, as well as medical manipulations. The provision of patronage care in the home will be ensured by recruiting and employing the necessary qualified medical and non-medical personnel, its training for the provision of services, purchase of means of transport, equipment, provision of materials and consumables and covering the costs of remuneration and insurance of employees, as well as occupational medicine. With the realisation of the project will ensure the provision of quality integrated health-social service for elderly and disabled people, which will achieve an integrated approach to care for elderly people in inability to self-service and persons with disabilities, with clear social impact and impact.     Проект "Патронажна грижа за възрастни хора и лица с увреждания в община Ловеч" ще бъде реализиран по процедура чрез директно предоставяне на БФП BG05M9OP001-2.040 "Патронажна грижа за възрастни и лица с увреждания- Компонент 2" в рамките на Приоритетна ос 2 "Намаляване на бедността и насърчаване на социалното включване" по ОП РЧР 2014-2020. Проектът предвижда предоставянето на нов вид услуга на територията на Община Ловеч, чрез изграждане на мрежа от здравно-социални услуги в домашна среда и е насочено, както към преките потребители- възрастни хора и лица с увреждания, вкл. с хронични заболявания и трайни увреждания, така и към лицата, ангажирани с предоставянето на грижите. Чрез реализирането на дейностите по проекта ще се подпомогнат лица, самотно живеещи в малките населени места и с невъзможност да се грижат за себе си, при изписване на пациенти от болница с необходимост от домашна медицинска грижа и хора с увреждания. Предоставянето на услугата патронажна грижа ще осигури посещения на социален асистент за подпомагане в ежедневието и дома и посещения на квалифицирано медицинско лице по домовете на потребителите, което ще облекчи необходимостта от пътуване за посещение до личен лекар и снабдяване с лекарства, както и извършване на медицински манипулации. Предоставянето на патронажни грижи в дома ще се обезпечи чрез набиране и назначаване на необходимия квалифициран медицински и немедицински персонал, неговото обучение за предоставяне на услугите, закупуване на транспортно средство, оборудване, осигуряване на материали и консумативи и покриване на разходи за възнаграждения и осигуровки на наетите лица, както и трудова медицина.С реализирането на проекта ще се осигури предоставянето на качествена интегрирана здравно-социална услуга за възрастни хора и лица с увреждания, с което ще се постигне прилагането на интегриран подход при грижата за възрастните хора в невъзможност за самообслужване и лицата с увреждания, с ясен социален ефект и въздействие.
