The project “Patronage Care in Krumovgrad Municipality” aims to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly by providing a network of services in the home environment and building appropriate material and personnel capacity to provide them. This will be done through the provision of part-time integrated health and social services at the homes of consumers, according to the program developed by the Ministry of Health (methodics) within the framework of the procedure “Patronage care for elderly people and persons with disabilities”. The idea for such upgrading of social services is based on the diverse and dynamic needs for support in everyday life of a large number of people with disabilities and elderly people, as well as on the need for effective and efficient provision of support in the context of limited resources. Users of the services under the project will be elderly people over 65 years of age with limitations or inability to self-service and people with disabilities and their families. Elderly people with chronic diseases and persons with permanent disabilities will be provided with specialised medical care in their homes. For the purposes of the project, staff will be recruited to manage services and staff to provide health-social services in a home environment. To ensure the integrated approach of services, staff will include experts with different competence profiles: social workers, medical professionals, rehabilitators, psychologists, nurses. In order to comply with the quality of service standards, healthcare professionals and specialists in the field of social services providing patronage care will receive specialised training organised by the Ministry of Health. To ensure equal access to the services under the project and a means of transport will be purchased for the purpose of carrying out the mobile work. The provision of integrated health-social services will be for a period of 12 months. (English)

    Проект "Патронажна грижа в община Крумовград" цели подобряване на качеството на живот и възможностите за социално включване на хората с увреждания и възрастните хора, чрез осигуряване на мрежа от услуги в домашна среда и изграждане на подходящ материален и кадрови капацитет за предоставянето им. Това ще стане чрез предоставянето на почасови интегрирани здравно-социално услуги по домовете на потребителите, съобразно разработена от Министерство на здравеопазването програма/методика/ в рамките на компонент 1 по процедура „Патронажна грижа за възрастни хора и лица с увреждания“.Идеята за подобно надграждане на социални услуги се основава на разнообразните и динамични потребности от подкрепа в ежедневието на голям брой хора с увреждания и възрастни хора, както и на необходимостта от ефективно и ефикасно предоставяне на подкрепа в условията на ограничени ресурси.Потребители на услугите по проекта ще бъдат възрастни хора над 65 г. с ограничения или в невъзможност за самообслужване и хора с увреждания и техните семейства. За възрастните хора с хронични заболявания и лицата с трайни увреждания ще се осигурят специализирани медицински грижи в техните домове. За целите на проекта ще бъдат назначени персонал за управление на услугите и персонал за предоставяне на здравно-социални услуги в домашна среда. За гарантиране на интегрирания подход на услугите персоналът ще включва експерти с различен профил на компетентността си: социални работници, медицински специалисти, рехабилитатори, психолог, санитари. С оглед спазване на стандартите за качество на услугата, медицинските специалисти и специалистите в областта на социалните услуги, предоставящи патронажната грижа ще преминат специализирано обучение, организирано от Министерство на здравеопазването. За осигуряване на равен достъп до услугите по проекта и ще бъде закупено транспортно средство с цел извършване на мобилната работа.Предоставянето на интегрирани здравно-социални услуги ще бъде за период от 12 месеца.
