По проекта ще бъде изградена мрежа от услуги в домашна среда и ще бъдат осигурени почасови мобилни здравно-социални услуги по домовете за лица с увреждания и възрастни хора над 65 г. в невъзможност за самообслужване. Част от средствата по проекта ще да бъдат използвани за закупуване на автомобил ,ремонт, обзавеждане и оборудване на помещенията, закупуване на два лаптопа и мултифункционално устройство,компютър с монитор,както и за обучение и супер визия на персонала. Операцията „Патронажна грижа за възрастни хора и лица с увреждания – Компонент 2“ надгражда модела за предоставяне на почасови мобилни интегрирани здравно-социални услуги за възрастни хора.
