The overall objective of the project is to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly by providing a network of services in the home environment and building appropriate capacity for their provision in the municipalities of Belitsa, Hadjidimovo and Garmen. The specific objectives of the project are:- to build a model for patronage care for elderly and disabled people, including chronic diseases and permanent disabilities in order to provide hourly mobile integrated health and social services in their homes. — to create three working social service providers with qualified staff and high standards, providing integrated services for people with disabilities and their families, people over 65 with limitations or inability to self-service, combining complex actions to provide long-term care, including social and health services in the community or in a home environment, according to the specific needs of the disadvantaged person on the basis of an individual assessment of needs.The project is aimed at a total of 75 persons from Belitsa, Hadjidimovo and Garmen — elderly people over 65 years old with restrictions or inability to self-service and people with disabilities and their families. The activities to be implemented under the project include the provision of hourly mobile integrated health and social services for deprived persons with disabilities and the elderly, training and supervision of staff; furnishing and equipping suitable premises, including the purchase of means of transport for carrying out mobile work. Represent the target groups involved in the project will increase their quality of life by accessing quality services in a home environment, according to their individual needs. Общата цел на проекта е подобряване на качеството на живот и възможностите за социално включване на хората с увреждания и възрастните хора, чрез осигуряване на мрежа от услуги в домашна среда и изграждане на подходящ капацитет за предоставянето им в общините Белица, Хаджидимово и Гърмен.Специфичните цели на проекта са:- да се изгради модел за патронажни грижи за възрастни хора и лица с увреждания, вкл. с хронични заболявания и трайни увреждания с цел осигуряване на почасови мобилни интегрирани здравно-социални услуги в техните домове. - да се създадат три работещи доставчици на социални услуги с квалифициран персонал и високи стандарти, чрез което да се предоставят интегрирани услуги за хора с увреждания и техните семейства, хора над 65 г. с ограничения или в невъзможност за самообслужване, като се съчетаят комплексни действия в посока осигуряване на дългосрочна грижа, вкл. и социални и здравни услуги в общността или в домашна среда, според специфичните потребности на човека в неравностойно положение на основата на индивидуална оценка на потребностите.Проекта е насочен към общо 75 лица от бщините Белица, Хаджидимово и Гърмен - възрастни хора над 65 г. с ограничения или в невъзможност за самообслужване и хора с увреждания и техните семейства. Дейностите които ще бъдат изпълени по проекта включват предоставяне на почасови мобилни интегрирани здравно-социални услуги за нуждаещи се лица с увреждания и възрастни хора, обучение и супервизия на персонала; обзавеждане и оборудване на подходящи помещения, включително и закупуване на транспортни средства за извършване на мобилната работа. Представителете на целевите групи, включени за участие в проекта ще повишат качеството си на живот, чрез достъп до качествени услуги в домашна среда , съобразно индивидуалните им потребности.
