The project proposal is aimed at improving the quality of life of the socially vulnerable and risk groups in Cherven briag Municipality, by providing integrated health and social services in the home environment and in the community in order to overcome social exclusion and poverty. The provision of quality part-time integrated services in the community and in the home environment is a mechanism for preventing the risk of falling into an institution. The project activities will create conditions for acquiring skills for independent living, tailored to the physical and social status of the users and will build on already implemented projects on the territory of OPHRD municipality, as overlapping the objectives of Measure 6 of the CSLLD of “Karnukovsky Karst — Cherven briag — Iskar” LAG and will respond to the need to provide adequate and specialised assistance to the target group. For the provision of the hourly services within the CSFSF, persons will be employed to serve those in need on the territory concerned. The provision of services will be based on the identified needs and problems and the individual social assessment of the target group representatives in need of constant care and inability to self-service. We foresee that 140 persons will be serviced under the project. The caretaker service will be provided in the homes of consumers. For a better quality provision of the services, we foresee the complex of social, health, psychological and rehabilitation services to be provided on an hourly basis, both at the CFCSP and at the homes of the consumers. “MHAT — Cherven briag” Ltd. has its own specialised car, which will be used to transport consumers from their homes to the CFCSP and vice versa (if they wish).All project activities are aimed at building a sustainable model for quality provision of health and social services on the territory of Cherven briag Municipality and are organised in a way that ensures optimal use of human and financial resources. Проектното предложение е насочено към подобряване качеството на живот на социално уязмимите и рискови групи в Община Червен бряг, чрез осигуряване на интегрирани здравно-социални услуги в домашна среда и в общността с цел преодоляване на социалното изключване и бедността. Предоставянето на качествени почасови интегрирани услуги в общността и в домашна среда е механизъм за превенция на риска от попадане в институция. Дейностите по проекта ще създават условия за усвояване на умения за независим живот, съобразени с физическия и социален статус на потребителите и ще надградят вече реализирани проекти на територията на общината по ОПРЧР, както припокриват целите на Мярка 6 от СВОМР на „МИГ Карлуковски карст – Червен бряг - Искър“ и ще отговори на необходимостта от предоставяне на адекватна и специализирана помощ на лицата от целевата група. За предоставяне на почасовите услуги в рамките на ЦРФСП ще бъдат наети лица, които да обслужват нуждаещите се на съответната територия. Предоставянето на услугите ще се основава на идентифицираните нужди и проблеми и индивидуалната социална оценка на представителите на целевата група, нуждаещи се от постоянна грижа и в невъзможност за самообслужване. Предвиждаме по проекта да се обслужват 140 лица. Услугата - домашен санитар ще бъде предоставяна в домовете на потребителите. За по-качествено предоставяне на услугите предвиждаме комплексът от социални, здравни, психологически и рехабилитационни услуги да се предоставят почасово, както в ЦРФСП , така и по домовете на потребителите. "МБАЛ - Червен бряг" ЕООД разполага със собствен специализиран автомобил, с който ще бъдат извозвани потребителите от домовете им до ЦРФСП и обратно (при тяхно желание).Всички проектни дейности са насочени към изграждане на устойчив модел за качествено предоставяне на здравно-социални услуги на територията на Община Червен бряг и са организирани по начин, гарантиращ оптимално използване на човешки и финансов ресурс.
