    OBJETIVO Evaluar y comparar en un modelo experimental de déficit de surfactante los cambios fisiológicos, bioquímicos e histológicos originados a nivel pulmonar, sistémico y cerebral, tras la administración de surfactante en forma bolus y en forma nebulizada durante ventilación invasiva y ventilación no invasiva (hi-flow). METODOLOGIA Diseño y sujetos de estudio: 6 grupos de cerdos neonatales (2-4 días de edad) con déficit de surfactante (n=8/grupo). De forma randomizada recibirán tratamiento (SF) en una de las diferentes formas de administración (bolus vs. nebulizado), mientras se administra ventilación invasiva o ventilación no invasiva. Cirugía: Los animales serán cateterizados (arteria femoral y vena yugular) y se aislara una carótida para la medición de flujo de forma continua a través de una sonda ultrasónica de flujo. Se determinara la perfusión y oxigenación cerebral mediante NIRS y de evaluara los cambios en el electroencefalograma. Se realizaran lavados broncoalveolares para conseguir la depleción del SF y posteriormente los animales serán randomizados al tratamiento correspondiente. Determinaciones: se medirán parámetros cardivasculares, intercambio gaseoso y función pulmonar. Del mismo modo, se medirá la hemodinámica, metabolismo y función cerebral. Después de la extracción de muestras se estudiara el daño histológico, el estrés oxidativo y la inflamación mediante diferentes técnicas bioquímicas e inmunoensayos. (Spanish)

    Objective To evaluate and compare in an experimental model of surfactant deficit the physiological, biochemical and histological changes originated at the pulmonary, systemic and cerebral level, after administration of surfactant bolus form and nebulised form during invasive ventilation and non-invasive ventilation (hi-flow). Methodology Design and subjects of study: 6 groups of neonatal pigs (2-4 days old) with surfactant deficit (n=8/group). Randomised treatment (SF) will be given in one of the different forms of administration (bolus vs. nebulised), while invasive ventilation or non-invasive ventilation is administered. Surgery: The animals will be catheterised (femoral artery and jugular vein) and a carotid is isolated for continuous flow measurement through an ultrasonic flow probe. The brain infusion and oxygenation will be determined by NIRS and the changes in the electroencephalogram will be evaluated. Bronchoalveolar washes will be carried out to achieve SF depletion and subsequently the animals will be randomised to the corresponding treatment. Determinations: cardiovascular parameters, gas exchange and lung function shall be measured. Similarly, hemodynamics, metabolism and brain function will be measured. After sampling, histological damage, oxidative stress and inflammation will be studied using different biochemical and immunoassay techniques. (English)
