This project will analyse the potential of circulating exosomes and microRNAs as biomarkers for the diagnosis and prognosis of progression in elite controlling patients and their possible use as a therapeutic target.     Los exosomas circulantes tienen un papel importante en muchos procesos celulares y en muchas patologías, albergando moléculas como los microRNAs que son candidatos muy prometedores para su uso como biomarcadores. Hay muchos trabajos sobre el papel de los miRNAs en el ámbito de la infección por el VIH-1 demostrándose que la expresión alterada de estas moléculas desempeña un papel fundamental en la enfermedad. Sin embargo, son muy pocos los estudios realizados en pacientes infectados por el VIH-1 que presentan un control natural de la infección en ausencia de tratamiento antirretroviral (controladores de élite), los cuales representan una prevalencia muy baja (1-5%). El presente proyecto analizará el potencial de los exosomas circulantes y los microRNAs como biomarcadores para el diagnóstico y pronóstico de progresión en pacientes controladores de élite y su eventual utilización como diana terapéutica. Además, se asociarán a parámetros inmunofenotípicos, de la inmunidad innata y adquirida, y de citocinas solubles. (Spanish)
