Property / summary: The influence of sleep apnea-hypoapnea syndrome (SAHS) and continuous air pressure treatment (CPAP) on circadian intraocular pressure (PIO) pattern and its structural impact on the retina nerve fiber layer (CFNR) will be evaluated in order to analyse the association between SAHS and glaucoma.OBJECTIVES: 1. Study circadian patterns of IOP for 24 hours in patients with SAHS prior to initiation of CPAP therapy. Objective 2. Assess the effect of CPAP on IOP. 3. Evaluate the effect of IOP changes in patients with CPAP-treated SAHS on CFNR. METHODOLOGY: Prospective randomised cross-controlled clinical trial in which the circadian pattern of IOP will be continuously monitored for 24 hours using the Sensimed Triggerfish contact lens device (Sensimed AG, Switzerland).Objective 1.IOP will be monitored for 24 hours to 74 SAHS patients prior to initiation of CPAP treatment and 37 without SAHS. Objective 2. The impact of CPAP treatment will be evaluated following a two-stage study design CPAP/crossplacebo. A first IOP monitoring will be compared prior to initiation of CPAP therapy, with monthly monitoring (CPAP/placebo) and 12 months after initiation of CPAP treatment.Objective 3. The correlation of the values obtained from IOP monitoring in the thickness of the CFNR, measured by optical coherence tomography, shall be evaluated at the beginning of the study and at 12 months after initiation of CPAP treatment. Se evaluará la influencia del síndrome de apnea-hipoapnea del sueño (SAHS) y del tratamiento con presión continua de aire (CPAP) sobre los patrón circadiano de la presión intraocular (PIO) y su repercusión estructural en la capa de fibras nerviosas de la retina (CFNR) para poder analizar la asociación entre SAHS y glaucoma.OBJETIVOS: 1. Estudiar los patrones circadianos de la PIO durante 24 horas en pacientes con SAHS antes de iniciar el tratamiento con CPAP. Objetivo 2. Evaluar el efecto de la CPAP sobre la PIO. 3. Evaluar el efecto de los cambios de la PIO en los pacientes con SAHS tratados con CPAP en la CFNR. METODOLOGÍA: Ensayo clínico prospectivo aleatorizado controlado cruzado en el que se monitorizará de forma continua el patrón circadiano de la PIO durante 24h mediante el dispositivo de lente de contacto Sensimed Triggerfish (Sensimed AG, Suiza).Objetivo 1.Se monitorizará la PIO durante 24 horas a 74 pacientes SAHS antes de iniciar el tratamiento con CPAP y a 37 sin SAHS. Objetivo 2. Se evaluará el impacto del tratamiento con CPAP siguiendo un diseño de estudio en dos etapas CPAP/placebo cruzado. Se comparará una primera monitorización de la PIO antes de iniciar el tratamiento con CPAP, con una monitorización al mes (CPAP/placebo) y a los 12 meses de haber iniciado el tratamiento con CPAP .Objetivo 3. Se evaluará la correlación de los valores obtenidos de la monitorización de la PIO en el grosor de la CFNR, medida por tomografía de coherencia óptica, al inicio del estudio y a los 12 meses del inicio del tratamiento con CPAP.
