El objetivo principal del proyecto es determinar el papel de la Reserva Cognitiva (RC) en la evolución de la Esclerosis Múltiple (EM). Metodología: Estudio observacional cohortes prospectivo en el que se reclutarán, durante dos años todos los pacientes atendidos en CCEE de neurología de los centros participantes. En una primera fase se evaluará, en un corte trasversal, si la RC se relaciona con la evolución de la EM en función de la actividad de la enfermedad, y discapacidad física o cognitiva. En la segunda fase, estos pacientes serán seguidos hasta dos años para evaluar la relación entre la RC y evolución de la EM, añadiendo la calidad de vida y el estado de ánimo en la definición de esa evolución. Se incluirán pacientes prevalentes y de nuevo diagnóstico con discapacidad que le permite caminar. Se analizarán variables clínicas, cognitivas, ansiedad, depresión, y calidad de vida que se evaluará en el momento de la inclusión en el estudio y a los dos años. Se realizarán análisis multivariantes para medir la asociación de la RC, así como de más variables de exposición, con la calidad de vida de los pacientes, mediante modelos lineales generalizados. Con este trabajo, contaremos con las mediciones basales necesarias para realizar en el futuro un estudio prospectivo que muestre el valor pronóstico de la RC en la evolución de pacientes con EM, con el fin de diseñar intervenciones dirigidas a potenciar/preservar la RC que puedan hacer el curso de la enfermedad más benigno. The main objective of the project is to determine the role of the Cognitive Reserve (CR) in the evolution of Multiple Sclerosis (MS). Methodology: Prospective observational cohort study in which all patients treated at the neurology CCEE of the participating centers will be recruited for two years. In a first phase, it will be assessed, in a transversal cut, whether the CR is related to the evolution of MS according to the activity of the disease, and physical or cognitive disability. In the second phase, these patients will be followed up to two years to evaluate the relationship between CR and MS evolution, adding quality of life and mood in the definition of this evolution. It will include prevalent and newly diagnosed patients with disabilities that allow you to walk. Clinical, cognitive, anxiety, depression, and quality of life variables will be analysed and evaluated at the time of inclusion in the study and at two years. Multivariate analyses will be performed to measure the association of CR, as well as more exposure variables, with patients’ quality of life, using generalised linear models. With this work, we will have the baseline measurements necessary to carry out in the future a prospective study that shows the prognostic value of CR in the evolution of patients with MS, in order to design interventions aimed at enhancing/preserving CR that can make the course of the disease more benign.
