Bronchopulmonary colonisation by Pseudomonas aeruginosa conditions the management of the patient with cystic fibrosis (CF). It is usually preceded by Staphylocccus aureus which persists (co-colonisation) or is displaced by P. aeruginosa. Colonisation by S. aureus could be of importance in these stages and in subsequent evolution according to the previous multicenter study PI12/00734 The current project will expand the knowledge of this pathogen in CF. The population structure of S. aureus in CF in Spain is not known, its relationship with resistance and virulence factors, operon agr dysfunction (involved in hetero-resistance and virulence) and there are no studies that analyse the dynamics of the microbiome in respiratory secretions and its involvement in different stages (primocolonisation, chronic colonisation and exacerbation). In this project it will address these aspects as objectives. Each CF Unit (7 pediatrics and 9 adults), distributed throughout Spain, will include 12 patients, differentiating colonisation status and clinical characteristics, with a one-year follow-up. Respiratory secretions and serums will be collected from each patient and sent to the coordinating centre (Hosp. Ramón y Cajal) for microbiological cultivation, detection of pathogens, PCR-real time, population study (PFGE, MLST), characterisation of mechanisms of resistance to antibiotics of choice in CF and virulence genes and markers of inflammation. In 15 selected patients, with (n=9) and without (n=6) colonisation by S. aureus, the microbiome and its colonisation dynamics of the respiratory mucosa will be studied by metagenomics (MiSeq) and bioinformatics. The results will be compared with those obtained in project PI12/00734 in patients colonised by P. aeruginosa. These results will serve for new diagnostic tools and clinical recommendations in patients colonised by S. aureus in CF. (English)

    La colonización broncopulmonar por Pseudomonas aeruginosa condiciona el manejo del paciente con fibrosis quística (FQ). Suele estar precedida por Staphylocccus aureus que persiste (co-colonización) o es desplazado por P. aeruginosa. La colonización por S. aureus podría tener importancia en estos estadios y en la evolución posterior según el estudio multicéntrico previo PI12/00734 El proyecto actual ampliará los conocimientos de este patógeno en FQ. No se conoce la estructura poblacional de S. aureus en FQ en España, su relación con la resistencia y factores de virulencia, disfunción del operon agr (implicado en heterorresistencia y virulencia) y no hay estudios que analicen la dinámica del microbioma en las secreciones respiratorias y su implicación en diferentes estadíos (primocolonización, colonización crónica y exacerbación). En este proyecto abordará estos aspectos como objetivos. Cada Unidad de FQ (7 pediátricas y 9 de adultos), distribuidas por toda España, incluirá 12 pacientes, diferenciando el estado de colonización y características clínicas, con un seguimiento de un año. Se recogerán secreciones respiratorias y sueros de cada paciente y enviarán al centro coordinador (Hosp. Ramón y Cajal) para cultivo microbiológico, detección de patógenos, PCR-tiempo real, estudio poblacional (PFGE, MLST), caracterización de mecanismos de resistencia a antibióticos de elección en FQ y genes de virulencia y marcadores de inflamación. En 15 pacientes seleccionados, con (n=9) y sin (n=6) colonización por S. aureus, se estudiará por metagenómica (MiSeq) y bioinformática el microbioma y su dinámica de colonización de la mucosa respiratoria. Los resultados se compararán con los obtenidos en el proyecto PI12/00734 en pacientes colonizados por P. aeruginosa. Estos resultados servirán para nuevas herramientas de diagnóstico y recomendaciones clínicas en pacientes colonizados por S. aureus en FQ. (Spanish)
