The intestinal microbiota is a complex ecosystem with enormous metabolic potential that contributes to the general state of health. A comprehensive study of microbiota, nutrition, inflammation and antibiotic therapies is proposed in critical patients with severe sepsis (acute infectious process) and subjects with Cystic Fibrosis (CF) (chronic infectious process). The objectives are 1) Analyse the composition and evolution of the intestinal microbiota of critical septic patients admitted to the Medical ICU according to the time of fasting and the type of feeding they receive, 2) Assess the effect of the early establishment of the diet on the critical septic patient from the point of view of bacterial translocation, immunological tolerance, and basal and intestinal inflammation, 3) Determine the composition and particularities of the intestinal microbiota of patients with CF in relation to their nutritional status and basal inflammation, analysing the possible relationship between the pulmonary microbiota and the intestinal profile of patients with CF, which allows for a new diagnosis of the intestinal tract through a diagnosis of the molecular disease and its basal inflammation. The methodology includes metagenomic techniques and pyrossequencing; and the integral analysis of all variables. The innovative contribution will allow in a simple, cheap and fast way to evaluate the correct functioning of the intestinal microbiome and to establish the metabolic diagnosis of dysbiosis. This is a translational project that will improve the quality of life of the septic patient. (English)

    La microbiota intestinal es un complejo ecosistema con enorme potencial metabólico que contribuye al estado general de salud. Se propone un estudio integral de microbiota, nutrición, inflamación y terapias antibióticas en pacientes críticos con sepsis severa (proceso infeccioso agudo) y sujetos con Fibrosis Quística (FQ) (proceso infeccioso crónico). Los objetivos son 1) Analizar la composición y la evolución de la microbiota intestinal de pacientes sépticos críticos ingresados en la UCI Médica según el tiempo de ayuno y el tipo de alimentación que reciban, 2) Evaluar el efecto de la instauración precoz de la alimentación en el paciente séptico crítico desde el punto de vista de la translocación bacteriana, la tolerancia inmunológica, y la inflamación basal e intestinal, 3) Determinar la composición y particularidades de la microbiota intestinal de pacientes con FQ en relación a su estado nutricional y su inflamación basal, analizando la posible relación entre la microbiota pulmonar y la intestinal de los pacientes con FQ, 4) Identificar los mecanismos moleculares de resistencia a antibióticos en la microbiota intestinal de pacientes sépticos críticos, 5) Diseñar un nuevo algoritmo que permita diagnosticar disbiosis a través del perfil metabólico en orina o plasma. La metodología incluye técnicas metagenómicas y pyrosecuenciación; y el análisis integral de todas las variables. La aportación innovativa permitirá de una forma sencilla, barata y rápida evaluar el correcto funcionamiento del microbioma intestinal así como establecer el diagnóstico metabólico de disbiosis. Este es un proyecto traslacional que permitirá mejorar la calidad de vida del paciente séptico. (Spanish)
