Due to their molecular characteristics, it is postulated that the study of miRNAs can contribute to definitively solve this problem. The main objective of this multicenter project (3 centers) is: define a differential algorithm of microvesicular miRNAs with diagnostic and prognostic capacity of septic shock. Correlate these results with those of our group’s previous transcriptome study (PI15/01451) and finally validate algorithms in an independent cohort. the methodology for the first two objectives will analyse the 120 samples (n=60 septic shock and n=60 non-septic shock) included in the previous work (PI15/01451). We will perform the isolation of the microvesicles by precipitation and ultrafiltration; quantification of miRNAs by mass quantification and performance assessment by Bioanalyser and quantitative PCR. The bioinformatics analysis will allow us to define the miRNAs algorithm representative of each arm and the validation will be performed in an independent cohort of 108 patients (n=36 septic shock, n=36 non-septic shock and n=36 controls), following the same experimental approach. The results obtained would provide definitive tools for accurate and early diagnosis of septic shock. It would also favor the involvement of biotechnology companies with the national health system since there is the possibility of patenting diagnostic kits, as endorsed by the document of business interest.     En España, la mortalidad en el caso de shock séptico es inaceptablemente elevada llegando a cifras 50-60% y aún no existe un “gold standard” para su diagnóstico. Los biomarcadores existentes pueden ayudan al diagnóstico clínico, pero en ningún caso ser recomendados como un test diagnóstico definitivo. Debido a sus características moleculares, se postula que el estudio de los miRNAs puede contribuir a solventar definitivamente este problema. El objetivo principal de este proyecto multicéntrico (3 centros) es: definir un algoritmo diferencial de miRNAs microvesiculares con capacidad diagnóstica y pronóstica de shock séptico. Correlacionar estos resultados con los del estudio del transcriptoma previo de nuestro grupo (PI15/01451) y por último, validar los algoritmos en una cohorte independiente. la metodología para los dos primeros objetivos analizaremos las 120 muestras (n=60 shock séptico y n=60 shock no séptico) incluidas en el trabajo previo (PI15/01451). Realizaremos el aislamiento de las microvesículas por precipitación y ultrafiltración; cuantificación de los miRNAs por cuantificación masiva y valoración de su rendimiento por bioanalyzer y PCR cuantitativa. El análisis bioinformático nos permitirá definir el algoritmo de miRNAs representativo de cada brazo y la validación la realizaremos en una cohorte independiente de 108 pacientes (n=36 shock séptico, n=36 shock no séptico y n=36 controles), siguiendo la misma aproximación experimental. Los resultados obtenidos aportarían herramientas definitivas para realizar diagnóstico preciso y precoz del shock séptico. También favorecería la implicación de empresas biotecnológicas con el sistema nacional de salud ya que existe la posibilidad de patentar kits diagnóstico, como avala el documento de interés empresarial.
