Projekt (P) dotyczy ułatwienia dostępu 60 (35K i 25M) osobom niesamodzielnym 60+ (ON) z powiatów: pruszkowskiego i grodziskiego do usług społ. (opiekuńczych i specj. usług opiekuńczych), świadczonych w społeczności lokalnej. Usługi świadczone będą w m. zam. ON i w ośrodku Wnioskodawcy (WN) w Milanówku, w którym zostanie uruchomiony Dzienny Dom Pobytu (DDP) dla 30ON. Dzięki zwiększeniu dostępu do tych usług możliwa będzie poprawa stanu zdrowia psych. i fiz. ON. Możliwe będzie pozostawienie ON w ich domach (deinstytucjonalizacja usług opiekuńczych) i funkcjonowanie we własnym otoczeniu wśród miejsc i ludzi, których znają. Opiekunom faktycznym (OF) P da szansę na łączenie opieki nad ON z realizacją w życiu zaw. i społ. Ww. usługi świadczone będą zgodnie z ustawą z 12.03.2004r. o pom. społ. Wsparcie P będzie również zgodne z „Ogólnoeuropejskimi wytycznymi dotyczącymi przejścia od (..,)", dokumentem „Wykorzystanie funduszy UE w celu przejścia od opieki instytucjonalnej (...)”, „Wytycznymi w zakresie realizacji przedsięwzięć w obszarze włączenia społ. …” oraz „Min. wymaganiami świadczenia (...)”. Organizacja wsparcia dla ON zostanie poprzedzone opracowaniem indyw. ścieżek wsparcia (IŚW), a zakończy się ich ewaluacją. Wsparcie będzie adresowane w pierwszej kolejności do ON,których dochód nie przekracza 150% kryterium dochodowego (na os. samodzielnie gospodarującą lub na osobę w rodzinie), o którym mowa w ustawie o pom. społ. W P premiowany będzie udział osób (os.) korzystających z POPŻ (nie będzie powielał wsparcia z tego programu) oraz udział osób niepełnosprawnych. W celu wsparcia OF w opiece nad ON, P zakłada w P: szkolenia z opieki nad ON (dla 20OF), ich wsparcie psychologiczne i poradnictwo dot. poruszania się po systemie wsparcia OF ON. Zakres wsparcia w P dla ON i ich OF doprowadzi do zwiększenia liczby os. zagrożonych ubóstwem lub wykluczeniem społ. objętych usługami społ. (60ON) oraz zwiększenia liczby miejsc świadczenia usł. społ. w społeczności lokalnej (34 (Polish)

Project (P) concerns the facilitation of access to 60 (35K and 25M) dependent persons 60+ (ON) from districts: Pruszkowski and Grodziski to the service of society. (caring and special care services), provided in the local community. Services will be provided in the city. He and the Applicant’s Centre (WN) in Milanówek, where the Daily Residence House (DDP) will be launched for 30ON. By increasing access to these services, it will be possible to improve the state of mental health and physical health. IT’S HIM. It will be possible to leave the SA in their homes (deinstitutionalisation of care services) and to operate in their own surroundings among the places and people they know. For actual caregivers (OF) P will give a chance to combine the care of the ON with the realisation in life of involving and social. The above services will be provided in accordance with the Act of 12.03.2004. The P support will also be in line with the "General European guidelines for the transition from (..,)", "Use of EU funds to move from institutional care (...)", "Guidelines for the implementation of projects in the field of social inclusion. ..." and "Min. requirements of provision (...)”. The organisation of support for the SA will be preceded by the development of Indigenous Support Paths (IMS) and will end with an evaluation. Support will be addressed in the first place to the SA, whose income does not exceed 150 % of the income criterion (for self-management or for a person in the family) referred to in the Act on Social Assistance. P will be rewarded for the participation of people (persons) benefiting from the APPZ (will not duplicate support from this programme) and participation of persons with disabilities. In order to support the OF in the care of the ON, P assumes in P: training on ON care (for 20OF), their psychological support and guidance on how to navigate the OF ON support system. The scope of support in P for SAs and their OFs will lead to an increase in the number of persons at risk of poverty or social exclusion covered by social services.
