Target group A. 72 biological families at risk of poverty or social exclusion, i.e. 258 os/162K (86odorosłe/73K and 172 dzieci/91K) benefiting from social assistance benefits. B. 28 Replacement Families (RZ), i.e. 89 os/44K (of which: 39 os./16K placed in family foster care and 50 persons from their entourage, i.e. members of these families, i.e. 42 osób/21K foster carers and 8 biological children for foster care. As a result of the project: 347os.at risk of poverty or exclusion will be supported by 8 social services of general interest in the programme, social services will be provided at the end of the programme, and 39 will be undermined.     Cel: Poprawa funkcjonowania 72 rodzin biologicznych (258 os./162K) zagrożonych ubóstwem lub wykluczeniem społecznym i 28 rodzin zastępczych (89os./44K, w tym: 39 dzieci umieszczonych w pieczy zastępczej zagrożonych ubóstwem lub wykluczeniem społecznym), zamieszkujących na terenie powiatu braniewskiego przez realizację zindywidualizowanych usług społecznych na rzecz rodzin w okresie 1.10.2017-31.12.2019. Zadania 1.Program wsparcia dla 72 rodzin biologicznych (86 osób dorosłych i 172 dzieci) - diagnoza, praca socjalna, asystentura rodzinna. 2.Program wsparcia rodzin biologicznych -ścieżka "Nowego życia" dla 11 rodzin (12 os. dorosłych i 24 dzieci). 3.Program wsparcia rodzin biologicznych - ścieżka „Zmiany w rodzinie” dla 56 rodzin (69os. dorosłych i 140 dzieci). 4.Program wsparcia rodzin biologicznych - ścieżka „Stop przemocy” dla 5 rodzin (5 os. dorosłych i 8 dzieci). 5.Program wsparcia dla 28 rodzin zastępczych (42 os. dorosłych i 47 dzieci). Grupa docelowa A. 72 rodzin biologiczne zagrożone ubóstwem lub wykluczeniem społecznym, tj. 258 os/162K (86os. dorosłe/73K i ich 172 dzieci/91K), korzystające ze świadczeń z pomocy społecznej. B. 28 Rodzin Zastępcze (RZ), tj. 89 os/44K (w tym: 39 os./16K umieszczone w rodzinnej pieczy zastępczej oraz 50 osoby z ich otoczenia, tj. członkowie tych rodzin, tj. 42 osób/21K sprawujących rodzinną pieczę zastępczą oraz 8 dzieci biologicznych os. sprawujących pieczę zastępczą. W wyniku realiz. projektu: 347os.zagrożonych ubóstwem lub wyklucz. społ. zostanie objętych usługami społecznymi świadczonymi w interesie ogólnym w programie, zostanie wspartych 8 miejsc świadczenia usług społecznych, istniejących po zakończeniu proj., 39 os. zagroż. wykl. społ. podejmie działania w zakr. poszukiwania pracy, podjęcia kształcenia lub szkolenia, zdobycia kwalifikacji, pracy.
