The project is part of the promoted reform in the field of Primary Health Care and concerns the operation of Local Health Groups (TOMY) in the Region of Eastern Macedonia – Thrace, consisting of health professionals and other staff, as defined in Article 106 of Law 4461/2017, and the assumption of responsibility by each WOM for the provision of holistic, human-centred and high quality health services, free and non-discriminatory. This project introduces for the first time the new structure of Primary Health Care Services, with the operation of the OEM. The TOMY is the basic unit for providing Primary Health Care services (prevention, promotion and health education) to individuals, families and the wider community, including vulnerable social groups, and is the core of the National Health System (NHS). The beneficiary of the project is the NSRF Executive Structure of the Ministry of Health, the Ministry of Health and Operation and Maintenance, the 4th Health Region of Macedonia and Thrace. Το έργο εντάσσεται στο πλαίσιο της προωθούμενης μεταρρύθμισης στον χώρο της Πρωτοβάθμιας Φροντίδας Υγείας και αφορά στη λειτουργία Τοπικών Ομάδων Υγείας (ΤΟΜΥ) στην Περιφέρεια Ανατολικής Μακεδονίας - Θράκης, αποτελούμενες από επαγγελματίες υγείας και λοιπό προσωπικό, σύμφωνα με τα οριζόμενα στο άρθρο 106 του Ν.4461/2017, και την ανάληψη ευθύνης από κάθε ΤΟΜΥ για την παροχή ολιστικών, ανθρωποκεντρικών και υψηλής ποιότητας υπηρεσιών υγείας, δωρεάν και χωρίς διακρίσεις. Με το έργο αυτό εισάγεται για πρώτη φορά η νέα διάρθρωση των Υπηρεσιών Πρωτοβάθμιας Φροντίδας Υγείας, με την λειτουργία των ΤΟΜΥ. Η ΤΟΜΥ αποτελεί τη βασική μονάδα παροχής υπηρεσιών Πρωτοβάθμιας Φροντίδας Υγείας (πρόληψη, προαγωγή και αγωγή υγείας) σε άτομα, οικογένειες και στην ευρύτερη κοινότητα, συμπεριλαμβανομένων των ευπαθών κοινωνικών ομάδων, και είναι ο πυρήνας του Εθνικού Συστήματος Υγείας (ΕΣΥ). Δικαιούχος του έργου είναι η Επιτελική Δομή ΕΣΠΑ του Υπουργείου Υγείας, Κύριος του έργου το Υπουργείο Υγείας και Φορέας Λειτουργίας και Συντήρησης, η 4η Υγειονομική Περιφέρεια Μακεδονίας και Θράκης.
