The main objective of the project is to improve or increase the social and/or professional competences of the participants by 31.12.2017. In the long term, aid to the group of 119 persons in difficulty or socially excluded from the site is planned. Participants will be supported by 5 tasks of active social, health, educational and vocational integration services depending on the identified problems and deficits of each participant. Projekt pn. Droga do zmiany - wsparcie osób zagrożonych wykluczeniem społecznym jest realizowany w partnerstwie przez następujące jednostki: Lidera tj. powiat pszczyński - Powiatowe Centrum Pomocy Rodzinie w Pszczynie oraz partnerów projektu: gmina Pszczyna - Ośrodek Pomocy Społecznej oraz Gmina Goczałkowice-Zdrój - Ośrodek Pomocy Społecznej. Projekt będzie realizowany w okresie od 1.01.2016 r. do 31.12.2017 r. na terenie powiatu pszczyńskiego w tym w gminie Pszczyna oraz Goczałkowice-Zdrój. Głównym celem projektu jest poprawa lub wzrost kompetencji społecznych i/lub zawodowych uczestnikom do 31.12.2017r. Docelowo zaplanowano pomoc grupie 119 osobom zagrożonym lub wykluczonym społecznie z w/w terenu. Uczestnicy będą mieli zapewnione wsparcie w ramach 5 zadań usług aktywnej integracji pod kątem społecznym, zdrowotnym, edukacyjnych, zawodowym w zależności od zdiagnozowanych problemów i deficytów każdego uczestnika z osobna. W grupie docelowej będą m.in. osoby niepełnosprawne i młodzież. (Polish)
