(Czech)

The project responds to the problem when people with Alzheimer’s disease who apply for admission to social care facilities (DZR) spend their waiting period that often exceeds 12 months in inappropriate conditions, because neither the family nor the normal care service is able to ensure their quality stay in the home environment. The project focuses on piloting the model, which allows these people to stay in their home environment as long as possible – motivates the family, raises professionals, fills gaps. (English)
