„Aktywnie Integrujący się Powiat Stargardzki” - to projekt realizowany na terenie powiatu stargardzkiego przez PCPR w Stargardzie. Okres realizacji: 01.10.2017 r. - 30.09.2019 r. Projektem objętych zostanie 95 osób – mieszkańców powiatu stargardzkiego i klientów PCPR w Stargardzie zagrożonych wykluczeniem społecznym z tytułu niepełnosprawności lub przebywania w pieczy zastępczej. Wsparciem objęte zostaną: 1. Dzieci i młodzież w pieczy - począwszy od wieku szkoły podstawowej, do momentu usamodzielnienia, wraz z osobami z otoczenia (w sytuacji gdy będzie to uzasadnione): 69 Uczestników Projektu (UP), w tym: 39K, 30M, 2. Osoby niepełnosprawne: 26 UP, w tym: 15K, 11M. Projekt zakłada wykorzystanie usług aktywnej integracji o charakterze: a) społecznym, b) edukacyjnym, c) zdrowotnym. Uczestnicy projektu zostaną objęci zindywidualizowaną ścieżką reintegracji (integracji społecznej/ indywidualnym programem usamodzielniania), obejmującą pracę socjalną oraz dopasowany do zdiagnozowanych na etapie rekrutacji deficytów, zestaw instrumentów aktywnej integracji (m.in. wyjazdy socjoterapeutyczne, warsztaty, doradztwo specjalistyczne). Rodziny zastępcze (otoczenie uczestników) objęte zostanie wsparciem podnoszącym kompetencje do pracy z dziećmi w pieczy w zakresie deficytów zdiagnozowanych u dzieci i młodzieży. Efektem realizowanych działań będzie poprawa różnych aspektów funkcjonowania uczestników w wymiarze społecznym, co dotyczyć będzie co najmniej 50% UP w odniesieniu do osób zagrożonych ubóstwem, a do osób o znacznym stopniu niepełnosprawności, osób z niepełnosprawnością intelektualną i osób z niepełnosprawnościami sprzężonymi 50%. “Actively Integrating Stargard County” is a project implemented in Stargard County by PCPR in Stargard. Period of implementation: 01.10.2017-30.09.2019. The project will cover 95 people – residents of the Stargard district and clients of PCPR in Stargard, who are at risk of social exclusion due to disability or being in foster care. Support will be provided for: 1. Children and adolescents in custody – from the age of primary school to the moment of emancipation, together with people from the environment (where justified): 69 Project Participants (UPs), including: 39K, 30M, 2. Persons with disabilities: 26 UP, including: 15K, 11M. The project involves the use of active inclusion services of the nature of: (a) social, (b) educational, (c) health. The participants of the project will be covered by an individualised path of reintegration (social integration/individual independence programme), including social work and tailored to deficits diagnosed at the recruitment stage, a set of active inclusion instruments (e.g. sociotherapeutic trips, workshops, specialist advice). Foster families (environmental surroundings) will be supported to improve competence to work with children in the care of deficits diagnosed in children and adolescents. The implementation of the actions will improve the various aspects of the social performance of participants, covering at least 50 % of PU for people at risk of poverty, and for people with severe disabilities, persons with intellectual disabilities and people with coupled disabilities of 50 %.
