Realizacja projektu obejmuje przebudowę i rozbudowę istniejącego budynku mieszkalnego z przeznaczeniem na Hospicjum Królowej Apostołów. Realizacja projektu w optymalnym zakresie zapewnia jego samowystarczalność w zakresie hospicyjnej działalności.Opiekę nad chorymi będzie sprawował etatowy personel medyczny, wspomagany przez wolontariuszy. Hospicjum będzie miejscem spotkań wspólnoty ludzi czynnie działających w ruchu hospicyjnym będzie tu również prowadzona działalność informacyjna i szkoleniowa dla rodzin chorych. Program funkcjonalny hospicjum obejmuje pokoje dla 19 chorych oraz zaplecze medyczne, sanitarne i kuchenne. Po zakończeniu realizacji projektu Hospicjum będzie gotowe do podjęcia działalności w zakresie stacjonarnej opieki paliatywnej, możliwe jest wykorzystanie w pełni infrastruktury i nie wymaga podjęcia dodatkowych działań. Wnioskodawca zapewni zasoby techniczne, kadrowe i wiedzę gwarantującą uruchomienie funkcjonowania infrastruktury bezpośrednio po zakończeniu projektu. (Polish)
