The project (type 1) is aimed at 80 people (50K) at risk of poverty and social exclusion, whether or not they are dependent, including disabled people living within the area of the “Gmina Choszno”, and their 20 ‘carers’ (15K). The main objective is to increase the number of people covered by services in the Gmina Choszo-80 community, including 50 women (hereinafter: (K) to meet the needs of these persons, including care services provided at home, Kor Senora, and to increase the knowledge of 20 guardians of fact (15K) in respect of their dependants in okr. 01.01.2020-31.10.2021. They respond to problems and needs in the supply of services which are identified in the area, taking into account trends in demograph trends and the level of availability of social services in the area. The proj will be directed to dependent persons, including the incomplete nose. (15 ON). Proj tasks: care support, assistant care., a centre of support to the actual carers, Kor Senora, contributing to the autonomy of the PA, increasing the availability and security of the people in the care sector. Services will be provided on the basis of high standards, by specialist staff, designed to provide these services, most of them at home. These services will serve to meet the growing needs arising from the inefficiency. 80 people do not. (50k) will be able to receive the necessary support outside the institutional system, so that they can be active in local communities for longer. Results: 1. The number of service points that exist after the end of the project: 13 (4 carers, 1 nurse, 1 AON, 1 rehabilitant, 1 psychologist, 5 Senior Citizens’ Club) 2.     Projekt (typ 1) jest skierowany do 80 osób (50K) zagrożonych ubóstwem i wykluczeniem społecznym, niesamodzielnych, w tym niepełnosprawnych, zamieszkałych na terenie woj. zach-pom w Gminie Choszczno oraz ich 20 opiekunów faktycznych (15K). Celem głównym proj jest zwiększenie liczby os objętych usług społ. w Gminie Choszczno o 80 os niesamodzielnych, w tym 50 kobiet (dalej: K), mającego służyć zaspokojeniu potrzeb tych os wynik. z niesamodzielności poprzez m.in. usługi opiekuńcze i asystenckie świadczone w miejscu zamieszkania, Klub Seniora oraz zwiększenie wiedzy 20 opiekunów faktycznych (15K) z zakr. opieki nad podopiecznymi w okr. 01.01.2020- 31.10.2021. Proj odpowiada na problemy i potrzeby w świadczeniu usług społ., zidentyfikowane na obszarze jego real. biorąc pod uwagę trendy demograf. i poziom dostępności usług społ. na tym obszarze. Proj będzie skierowany do os niesamodzielnych, w tym niepełnos. (15 ON). Zadania w ramach proj: wsparcie opiekuńcze, asystenckie, opiekuńcze specjal., centrum wsparcia opiekunów faktycznych, Klub Seniora, co przyczyni się do zwiększenia autonomii UP, zwiększenie dostępności i bezpieczeństwa dla podopiecznych. Usługi będą świadczone w oparciu o wysokie standardy przez specjalistyczną kadrę, przygotowaną do świadczenia tych usług, w większości w miejscach zamieszkania. Usługi te będą służyć zaspokojeniu rosnących potrzeb wynikających z niesam. Dzięki proj 80 osób niesam. (50K) będzie mogło otrzymać niezbędne wsparcie poza systemem instytucjonalizowanej pomocy, przez co dłużej będą mogły być aktywne w społeczności lokalnej. Rezultaty: 1. Liczba miejsc świadczenia usług społ. istniejących po zakończeniu projektu: 13 (4 opiekunki, 1 pielęgniarka, 1 AON, 1 rehabilitant, 1 psycholog, 5 lokalizacji Klubu Seniora) 2.
