The main goal of the project will be to create places of residence in a newly created assisted apartment – training. As a result, services will be provided to prepare persons residing in it for independent or minimal support to live in an open environment and to protect them from social exclusion. The target group of the project will consist of people or families at risk of poverty or social exclusion, e.g. persons with intellectual disabilities, people with mental disorders and homeless people, single parents raising children in difficulty. The project assumes three tasks. The main tasks to be carried out are: adaptation and renovation works of apartments of 72.69 m², equipment of the above-mentioned flats lent by the Municipality Gryfino, as well as actions for the benefit of participants, through the employment of a trainer and guardian. We assume that 24 people will benefit from the proposed actions throughout the project. GMINA GRYFINO/ OŚRODEK POMOCY SPOŁECZNEJ W GRYFINIE

Głównym celem projektu będzie utworzenie miejsc pobytu w nowo tworzonym mieszkaniu wspomaganym - treningowym. W rezultacie będą świadczone usługi przygotowujące osoby w nim przebywające do samodzielnego lub przy minimalnym wsparciu życia w otwartym środowisku oraz ochrona ich przed wykluczeniem społecznym. Grupę docelową projektu będą stanowiły osoby lub rodziny zagrożone ubóstwem lub wykluczeniem społecznym, m.in. osoby niepełnosprawne intelektualnie, osoby z zaburzeniami psychicznymi oraz osoby bezdomne, rodzice samotnie wychowujący dzieci znajdujący się w trudnej sytuacji. W ramach projektu zakłada się realizację trzech zadań. Głównymi zadaniami, które zostaną zrealizowane to: prace adaptacyjno - remontowe mieszkania o powierzchni 72,69 m2, wyposażenie w/w mieszkania użyczonego przez Gminę Gryfino, a także działania na rzecz uczestników, poprzez zatrudnienie trenera i opiekuna. Zakładamy, że przez cały okres realizacji projektu z proponowanych działań skorzystają 24 osoby. Uczestnicy projektu zostaną objęci kompleksowym wsparciem m.in. poprzez trening samodzielności, pracę socjalną, poradnictwo i inne usługi aktywnej integracji. (Polish)
