The project is implemented in partnership with Jastrowie commune and non-governmental organisation – AKME Foundation. The non-governmental partner was selected in accordance with Article 33 of the Act of 11 July 2014 on the principles for implementing cohesion policy programmes financed in the financial perspective 2014-2020 (hereinafter: the Implementation Act). The project will be supported by a total of 235 natural persons who, according to the KC, live, learn or work in the area of the Golden Poviat (Wielkopolskie Voivodeship). Project participants (except foster parents) are people at risk of poverty or social exclusion as defined in the guidelines on social inclusion, including: children, young people, elderly and disabled, families with children. The individual project activities will also involve the participants’ environment (which depends on the possibility of implementing actions and increasing their effectiveness). A total of 120 participants (90 persons – surrogate parents -34, those raised in foster care -41 and 15 biological children of the RZ as well as 30 disabled/independent persons), MGOPS Jastrów will support at least 30 disabled/independent people. The AKME Foundation will support the project towards 85 people: seniors, disabled/independent people, members of families experiencing caring and educational difficulties. The project will provide the following services: assisting support + accompanying services, social work and specialised counseling, mentoring, key competence courses, providing supported housing and animation activities in local communities. Projekt realizowany w partnerstwie z gminą Jastrowie oraz organizacją pozarządową - Fundacją AKME. Partner pozarządowy wyłonionyzostał zgodnie z art 33 ustawy ustawy z dnia 11 lipca 2014 r. o zasadach realizacji programów w zakresie polityki spójności finansowanych w perspektywie finansowej 2014-2020 (dalej: ustawa wdrożeniowa). Wsparciem projektu objętych zostanie łącznie 235 osób fizycznych, które zgodnie z KC zamieszkują, uczą się lub pracują na terenie powiatu złotowskiego (woj.wielkopolskie). Uczestnicy projektu (z wyjątkiem rodziców zastępczych) to osoby zagrożone ubóstwem lub wykluczeniem społecznym zgodnie z definicją zawartą w wytycznych w obszarze włączenia społecznego, w tym: dzieci, młodzież, osoby starsze i z niepełnosprawnościami, rodziny z dziećmi . W poszczególnych działaniach projektu udział weźmie również otoczenie uczestników (od czego uzależniona jest możliwość wdrożenia działań oraz zwiększenie ich skuteczności). PCPR Złotów (Partner wiodący) obejmie wsparciem łącznie 120 uczestników (90 osób - rodziców zastępczych -34, osób wychowujących się w pieczy zastępczej -41 oraz 15 dzieci biologicznych RZ a także 30 osób niepełnosprawnych/niesamodzielnych), MGOPS Jastrowie obejmie wsparciem min. 30 osób niepełnosprawnych/niesamodzielnych. Fundacja AKME wsparcie projektu kierować będzie w stronę 85 osób: seniorów, osób niepełnosprawnych/niesamodzielnych, członków rodzin przeżywających trudności opiekuńczo-wychowawcze. W ramach projektu świadczone będą następujące usługi: wsparcie asystenckie + usługi towarzyszące, praca socjalna i poradnictwo specjalistyczne, mentoring, kursy kompetencji kluczowych, zapewnienie mieszkania wspieranego a także działania animacyjne w lokalnych społecznościach.
