Celem niniejszego projektu jest wzrost dostępu do wysokiej jakości usług zdrowotnych w zakresie opieki nad kobietami w ciąży (w tym również z grup zagrożonych wykluczeniem społecznym i ubóstwem) i dziećmi w okresie prenatalnym w Sosnowcu i miastach ościennych do 31.01.2020 r. oraz poprawa jakości życia w skutek poprawy zdrowia i zmniejszenie nierówności w zdrowiu poprzez realizację projektu polegającego na objęciu 839 kobiet (100%K;0%M) w wieku poniżej 35 roku życia opieką perinatalną. Ponadto przewidziano realizację modułu przekrojowego poprzez działania edukacyjne dla personelu medycznego Wnioskodawcy, w tym POZ 20 os (12%Ki8%M).Projekt będzie realizowany w okresie od 01.08.2018 r. do 31.01.2020 r.
