Cel projektu skupia się na zwiększeniu dostępności wysokich jakościowo, zdeinstytucjonalizownaych usług zdrowotnych, zapewniających sprawność osobom narażonym na wykluczenie społeczne. Zapewni to im ochronę przed wykluczeniem oraz możliwość realnego, sprawnego funkcjonowania w lokalnej społeczności. Grupą docelową projektu są osoby wykluczone/ zagrożone ubóstwem lub wykluczeniem społecznym oraz ich otoczenie. Cel ma zostać osiągnięty dzięki kompleksowym procesom rehabilitacyjnym, przeznaczonym dla osób chorujących na stwardnienie rozsiane oraz pacjentów po udarach niedokrwiennych mózgu.
